营养支持疗法对老年原发性支气管肺癌患者化疗期间生活质量的影响

Influence of nutrition support therapy on quality of life in elderly patients with primary lung cancer during chemotherapy

目的分析营养支持疗法对提高老年原发性支气管肺癌患者化疗期间生活质量的效果。方法选择2011年5月-2013年7月海南省农垦总医院治疗的232例老年原发性支气管肺癌患者。根据治疗方式的不同将患者分为治疗组（132例）和对照组（100例）。所有患者均遵医嘱接受治疗,治疗组患者增加营养支持治疗,根据病情程度,选择肠内或肠外营养支持,每人保证200 g/d的葡萄糖供给,同时根据体重摄入蛋白质。化疗开始治疗3个月后,采用欧洲癌症研究与治疗组织癌症患者生活质量测定量表EORTC-QLQ-C30和EORTC-QLQ-LC13评定患者生活质量。结果与对照组比较,治疗组肺癌患者化疗期间躯体功能提高,疲乏和恶心呕吐有所缓解,差异均有统计学意义（P〈0.05）;肺癌患者化疗期间情绪、失眠症状、呼吸困难、口腔疼痛、脱发、胸痛等均有所改善,差异均有统计学意义（P〈0.05）。结论对老年原发性支气管肺癌患者实施营养支持疗法能提高患者生活质量,值得临床推广使用。

Objective To analyze the effect of nutrition supports therapy on improving the quality of life in elderly patients with primary lung cancer during chemotherapy. Methods From May 2011 to July 2013, in Hainan Agricultural Reclamation General Hospital, 232 elderly patients with primary lung cancer were selected. According to the different methods of treatment, the patients were divided into nutritional support therapy group（n = 132） and control group（n =100）. All patients followed prescribed therapy, based on this, the therapy group were given addional nutritional support,according to progression of disease, whether enteral or parenteral nutrition support were chosen, with each patient guaranteed 200 g/d glucose supply, and protein intake based on weight. 3 months after the chemotherapy treatment, the quality of life of cancer patients were evaluated by using EORTC-QLQ-C30 and EORTC-QLQ-LC13. Results Compared with the control group, physical function in nutritional support therapy group during chemotherapy improved, fatigue, nausea and vomiting eased, the differences were statistically significant（P 〈0.05）; mood and insomnia symptoms,difficulty breathing, sore mouth, hair loss and chest pain in lung cancer patients during chemotherapy improved, the differences were statistically significant（P〈 0.05）. Conclusion Implementation of nutrition support therapy in patients with primary lung cancer of elderly can improve quality of life, it is worth to use in clinical.
