前臂游离皮瓣移植修复口腔癌和口咽癌根治术后组织缺损临床分析

Clinical analysis of forearm flap ransplantation on reconstruction of tissue defects of oral and oropharynx cancer radical excision

目的探讨前臂游离皮瓣移植修复口腔癌和口咽癌根治术后组织缺损的临床效果。方法选择68例口腔癌和口咽癌患者,采用随机数表法分为对照组与观察组,每组各34例,观察组患者在口腔癌和口咽癌根治术同期采用前臂游离皮瓣移植修复组织缺损,对照组患者在口腔癌和口咽癌根治术同期采用胸大肌皮瓣修复组织缺损,比较两组患者皮瓣面积,皮瓣存活率及并发症发生率。结果两组均顺利完成手术,观察组患者皮瓣面积为(11.2±3.56)cm2,对照组为(11.9±4.17)cm2,两组皮瓣面积比较差异无显著性(P>0.05);观察组患者皮瓣存活率为97.1%,对照组为94.1%,两组比较差异无显著性(P>0.05);观察组患者并发症发生率为8.8%,低于对照组(26.5%),差异具有显著性(P<0.05)。结论前臂游离皮瓣移植修复口腔癌和口咽癌术后组织缺损并发症发生率低,可改善临床治疗效果,适用于口腔和口咽部恶性肿瘤术后组织缺损的修复。

Objective To investigate the clinical effect of forearm flap ransplantation on reconstruction of tissue defects of oral cancer and oropharynx cancer radical excision. Method 68 cases of patients with oral cancer and oropharynx cancer were selected and randomly divided into observation group and control group, 34 cases in each group, tissue defects of oral cancer or oropharynx cancer radical excision were rebuilded with forearm flap at the same time in observation group and with pectoralis major myocutaneous flap in control group, flap area, survival rate and complication were contrasted between the two groups. Result Flap area was（11.2±3.56） cm^2 in observation group and（11.9±4.17） cm^2 in control group, there was no significant difference between the two groups（P〉0.05）, survival rate of flap was 97.1% in observation group and 94.1% in control group, there was no significant difference between the two groups（P〉0.05）, incidence rate of complication was 8.8% in observation group, lower than control group（26.5%）, the difference was significant（P〈0.05）. Conclusion The forearm flap transplantation for repair of oral cancer and oropharyngeal cancer postoperaive tissue defect with lower complication rate, can improve clinical effect, eligible in reconstruction of tissue defects of oropharynx cancer radical excision.
