快速康复外科理念在腹腔镜胆囊切除术围手术期护理中的应用

Application to Concept of Rapid Rehabilitation Surgery in Laparoscopic Cholecystectomy Aroand Operation Period Nursing

目的探讨快速康复外科理念在腹腔镜胆囊切除术围手术期护理中的临床应用效果。方法选择2013年5月—2014年8月间我院收治的需进行腹腔镜胆囊切除术60例患者,按照临床围手术期护理方法将以上收录患者分为观察组及对照组,每组各30例患者。其中对照组患者于临床腹腔镜胆囊切除术围手术期均行常规护理,观察组患者于临床腹腔镜胆囊切除术围手术期均应用快速康复外科理念指导下的护理模式进行护理,对比两组患者的临床护理效果。结果观察组30例患者的平均排气时间约为28.6h,平均进食时间约为9.8h,平均下床时间为6.3h,平均住院天数为5.2d,共有2例患者发生术后并发症。其临床术后严重疼痛患者2例、一般疼痛患者17例、轻微疼痛患者11例;相比较对照组30例患者的平均排气时间约为39.4h,平均进食时间约为46.1h,平均下床时间为12.7h,平均住院天数为8.6d,共有7例患者发生术后并发症。其临床术后严重疼痛患者8例、一般疼痛患者20例、轻微疼痛患者2例。即观察组患者的临床护理效果显著优于对照组患者的临床护理效果(P<0.05)。结论将快速康复外科理念应用到腹腔镜胆囊切除术围手术期护理中的临床应用效果显著,能够更为有效地促进患者的临床术后恢复、减少患者术后并发症的发生率及减轻患者术后的疼痛,值得临床进一步推广研究。

Objective To explore the concept of fast track surgery in laparoscopic cholecystectomy clinical application effect of perioperative nursing care of. Methods Select 2013 May-2014 August treated 60 patients in our hospital from the need for laparoscopic cholecystectomy, according to clinical nursing methods in perioperative period of above will be included patients were divided into observation group and control group, each group had 30 patients. The patients in the control group in clinical peri operative period of laparoscopic cholecystectomy underwent conventional nursing, the observation group patients in clinical peri operative period of laparoscopic cholecystectomy were application of rapid rehabilitation nursing mode surgery under the guidance of the concept of nursing,clinical nursing effect of the two groups were compared. Results The average time to exhaust the observation group of 30 patients was about 28.6h, the average feeding time is about 9.8h, the average bed time is 6.3h, the average hospital stay was 5.2d, a total of 2 patients had postoperative complications. The clinical severe postoperative pain in 2 patients, 17 cases of patients with general pain, slight pain in11 patients; the average exhaust timecompared to a control group of 30 patients was about 39.4h, the average feeding time is about 46.1h,the average bed time is 12.7h, the average hospital stay was 8.6d, a total of 7 patients withpostoperative complications after. 2 cases of the8 cases, patients with severe pain clinical postoperative 20 cases, patients are generally minor aches and pains of patients with pain. The observation group patients clinical nursing effect of clinical nursing effect of significantly better than the control group patients（ P 〈0.05）. Conclusion The concept of rapid rehabilitation surgery applied tolaparoscopic cholecystectomy in perioperative nursing care of clinical application effect is remarkable, can more effectively promote theclinical patients after recovery, the incidence rate and reduce postoperative pain reduce postoperative complications, is worthy of further promotion research.
