超声引导下麦默通微创旋切系统与开放手术治疗乳腺良性肿块的疗效比较

Effect Comparison On Ultrasound Guided Mammotome Minimally Invasive System and Open Operation for the Treatment of Benign Breast Mass

目的比较超声引导下麦默通微创旋切系统与开放手术治疗乳腺良性肿块的临床疗效,为手术方案的制定提供参考。方法选取在我院接受超声引导下麦默通微创旋切术治疗的乳腺良性肿块患者600例作为观察组,选取同期在我院接受开放手术治疗的乳腺良性肿块患者300例作为对照组。对照组采用常规乳腺良性肿块开放手术进行治疗,观察组采用麦默通微创旋切术进行治疗。比较两组患者手术时间、术中出血量、术口愈合时间、瘢痕长度以及术后并发症的发生情况,同时比较两组患者对手术效果的满意程度。结果观察组的手术时间为(17.42±5.32)min,术中出血量为(3.98±1.27)ml,术口愈合时间为(3.83±1.08)d,手术瘢痕长度为(2.61±0.73)mm,均显著少于对照组(P均<0.05);观察组局部血肿的发生率为2.83%,未发生局部感染、乳腺畸形;对照组局部血肿的发生率为7.33%,局部感染的发生率为4.33%,乳腺畸形的发生率为2.33%。观察组患者局部血肿、局部感染以及乳腺畸形的发生率均显著低于对照组(P均<0.05);观察组的非常满意率为65.33%,显著高于对照组(P<0.05)。结论超声引导下麦默通微创旋切术具有手术创伤小、手术时间短、术后康复快及安全性高等优势,是治疗乳腺良性肿块较为理想的手术方式。

OBJECTIVE To compare the clinical efficacy of ultrasound guided mammotome minimally invasive system and open operation for the treatment of benign breast mass, and to provide a reference for the formulation of operation scheme. METHODS 600 patients received ultrasound guided mammotome minimally invasive surgery for benign breast masses were selected as the observation group, underwent open operation for the treatment of benign breast lumps of 300 patients in sample hospital as control group. The control group using conventional open operation for the treatment of benign breast mass, the observation group with mammotome minimally invasive surgery treatment. The two groups were compared operative time, bleeding volume, postoperative wound healing time, scar length and the postoperative complications were compared between two groups, and the surgical results of satisfaction degree. RESULTS The operation time of the observation group was （17.42±5.32） min, the amount of hemorrhage of （3.98±1.27） ml, wound healing time was （3.83± 1.08） d and operation scar length （2.61 ±0.73） mm were significantly less than that of the control group（P〈0.05）; observation group local hematoma occurred in 2.83%, no local infection, breast deformity; the control group of lo- cal hematoma occurred in 7.33%, local infection rate was 4.33%, the incidence rate of 2.33% breast deformity. The observation group local hematoma, local infection and breast deformity incidence was significantly lower than that of control group（P〈0.05）; observation group was very satisfactory rate was 65.33%, significantly higher than that in control group（P〈0.05）. CONCLUSION Ultrasound guided mammotome minimally invasive surgery with surgical trauma, short operative time, postoperative rehabilitation and security and other advantages were the ideal operation methods for treatment of benign breast mass.
