某院住院药房退药现状与防范措施分析

Status Quo of Drug Withdrawal in Inpatients Pharmacy of a Hospital

目的分析住院患者退药原因。方法收集某院2013年住院患者退药申请单21 900份,按临床科室、退药种类进行统计和分析。结果退药主要集中在内科、外科、重症监护室(ICU)、儿科和肿瘤科,共占退药总量的89.34%。退药原因主要为患者出院、转院和死亡,医嘱调整,患者拒绝用药,药品不良反应等。结论应加强药品管理,减少退药,保证临床用药安全、有效。

Objective To analyze the reasons for the drug withdrawal in the inpatients pharmacy. Methods A total of 21 900 application forms for drug withdrawal in the inpatients pharmacy of a hospital during 2013 were collected and performed the statistical analysis according to different clinical departments and varieties of returning drugs. Results Drug withdrawal was mainly concentrated in the departments of internal medicine,surgery,ICU,pediatrics and oncology,which accounted for 89. 34% of the drug withdrawal constituent ratio. The causes of drug withdrawal were mainly the patients discharge from hospital,transferring to another hospital and death,medical order adjustment,patient＇s refusal and adverse drug reaction,etc. Conclusion It is necessary to strengthen the drug management so as to reduce drug withdrawal and ensure safe and effective medication in clinic.
