摘要
目的 探讨胃癌组织中膜型基质金属蛋白酶-2(MT2-MMP)的表达水平与预后的关系.方法 采用免疫组织化学染色法检测胃癌组织芯片中90例癌组织及配对的癌旁组织中MT2-MMP的表达.采用x2检验分析MT2-MMP表达与临床病理特征的关系;采用Kaplan-Meier法及Log-rank检验分析MT2-MMP表达与总生存期(OS)的关系;拟合多因素Cox模型,采用风险比(HR)及95%可信区间(CI)评价不同临床指标与死亡的相关性.结果 MT2-MMP在胃癌组织中的表达高于癌旁组织(H-score评分均值分别为246和139分,P<0.01);胃癌组织中MT2-MMP的表达水平和患者的性别、年龄、肿瘤大小、病理分级、TNM分期无明显相关(P>0.05);单因素和多因素回归分析均显示MT2-MMP高表达胃癌患者的死亡风险较低表达组显著增加(HR=2.185,95% CI:1.175-4.064,P<0.05;HR=1.997,95% CI:1.042 -3.830,P<0.05).结论 MT2-MMP高表达提示胃癌患者预后较差.
Objective To investigate the expression of membrane type-2 matrix metalloproteinase (MT2-MMP) in gastric cancer and its correlation with prognosis.Methods The expression of MT2-MMP was detected by immunohistochemistry (IHC) in tissue microarrays containing paired tumor and peritumoral tissues from 90 patients who had undergone gastric resection for histologically proven gastric cancer.Chi-square test was utilized to analyze the relationship between MT2-MMP expression and clinicopathological features.Overall survival (OS) of different clinicopathological characteristics was compared using Kaplan-Meier and Log-rank test.The Cox proportional hazards model was performed to estimate hazard ratio (HR) with 95% confidence interval (CI) of the linking strength between the different clinical characteristics,expression of MT2-MMP and death risks.Results The MT2-MMP expression level was higher in tumor tissues than in peritumor tissues (the mean H-scores were 246 and 139 respectively,P 〈 0.01).No significant correlation was found between the MT2-MMP expression and gender,age,tumor size,pathological stage,TNM stage (all P 〉 0.05).Both univariate and multivariate analyses demonstrated that high MT2-MMP expression was a negative prognostic indicator of OS (HR =2.185,95% CI:1.175-4.064,P 〈 0.05 ;HR =1.997,95% CI:1.042-3.830,P 〈 0.05,respectively).Conclusion High expression of MT2-MMP was associated with poor prognosis.
