家访对居家腹膜透析患者容量控制的干预研究

Intervention Study on the Volume Control for Peritoneal Dialysis Patients with Follow-up Visit

目的探讨家访对居家腹膜透析患者容量控制行为的影响。方法通过电话家访、QQ家访和登门家访方式,对笔者所在科室住院治疗过的40例居家腹膜透析患者进行干预。结果通过6个月干预,出现2级高血压以上及心力衰竭等并发症的较少,腹膜透析患者体重较干预前明显减轻;血压、血清钠较干预前明显下降;腹膜透析超滤、尿量及血清白蛋白指标较干预前均明显提高,差异均有统计学意义(P<0.05)。结论家访提高了居家腹膜透析患者容量状况评估及处理容量超负荷的能力,降低了心血管疾病发生率。

Objective To investigate the effect of follow-up visit on volume control for peritoneal dialysis patients. Methods In total 40 patients with home peritoneal dialysis who had hospitalized in our department were included to be intervened with telephone follow-up, QQ follow-up and home visiting. Results After 6-month intervention, complications like hypertension above grade 2 and heart failure were rarely arisen. Peritoneal dialysis patients＇ weight was obviously lighter than before. Their blood pressure and blood serum sodium also reduced distinctly. The peritoneal dialysis ultrafiltration, urine volume and serum albumin were increased obviously with statistically significant difference（p0.05）. Conclusion Follow-up visit could not only improve ability of estimating volume status and treating the overloaded volume for patients with home peritoneal dialysis but also reduce the incidence rate of cardiovascular disease.
