不同含漱液预防系统性红斑狼疮患者口腔与肺部感染的临床研究

Clinical effects of different gargles on prevention of oral and pulmonary infections in patients with systemic lupus erythematosus

目的探讨黄芩、庆大霉素+制霉菌素及碳酸氢钠3组不同含漱液对于预防系统性红斑狼疮活动期患者口腔及肺部感染的临床对比分析,为临床诊断治疗提供依据。方法将医院138例系统性红斑狼疮活动期患者按照随机数字表法将其分为黄芩漱口组、庆大霉素+制霉菌素组、碳酸氢钠组,每组各46例;黄芩漱口组给予黄芩漱口液,庆大霉素+制霉菌素组给予庆大霉素8万U及4片制霉菌素200万U,碳酸氢钠组给予2.5%碳酸氢钠漱口。结果患者口腔真菌感染率黄芩漱口组8.70%、庆大霉素+制霉菌素组13.04%、碳酸氢钠组47.83%,3组比较差异有统计学意义(P<0.05);黄芩漱口组患者口腔真菌感染及肺部感染与庆大霉素+制霉菌素组、碳酸氢钠组患者比较,差异有统计学意义(P<0.05);黄芩漱口组患者口腔真菌感染及肺部感染出现的天数及治愈天数与庆大霉素+制霉菌素组、碳酸氢钠组患者比较,差异有统计学意义(P<0.05)。结论黄芩漱口组可明显有效减少系统性红斑狼疮活动期患者肺部感染及口腔感染发生率。

OBJECTIVE To explore the clinical effects of the skullcap mouthwash ,gentamicin plus nystatin ,and baking soda gargle on prevention of the oral and pulmonary infections in the patients with lupus erythematosus so as to provide guidance for clinical diagnosis and treatment .METHODS A total of 138 patients with active systemic lupus erythematosus were randomly divided into the skullcap mouthwash group , the gentamicin plus nystatin group ,and the baking soda group ,with 46 cases in each .The skullcap mouthwash group was treated with skull-cap mouthwash ,the gentamicin plus nystatin group was treated with 80 000 U of gentamicin plus 4 nystatin 2 , 000 ,000 U ,and the baking soda group was given 2 .5% sodium bicarbonate gargle .RESULTS The incidence of oral fungal infections was 8 .70% in the skullcap mouthwash group ,13 .04% in the gentamicin plus nystatin group , 47 .83% in the baking soda group ,there was significant difference between the three groups （P 〈 0 .05） ;there was significant difference in the incidence of oral and pulmonary fungal infections between the skullcap mouthwash group and the gentamicin plus nystatin group and the baking soda group （P〈 0 .05） ;there was significant differ-ence in the duration of oral and pulmonary fungal infections or cure duration between the skullcap mouthwash group and the gentamicin plus nystatin group and the baking soda group （P〈 0 .05） .CONCLUSION The skullcap mouthwash can significantly reduce the incidence of pulmonary and oral infections in the patients with active sys -temic lupus erythematosus .
