乳腺癌患者术后皮瓣下积液伴感染两种处理方式的效果比较

Comparison of effects of two treatments for postoperative infraskin-flap hydrops accompanied by infection in patients with breast cancer

目的比较引流管接负压球引流和纱布填塞引流两种方式处理乳腺癌术后皮瓣下积液伴感染治疗效果,为临床诊治提供理论依据。方法选择2011年3月-2014年3月医院收治乳腺癌术后发生皮瓣下积液伴感染患者30例,将其随机分为试验组15例和对照组15例,两组患者均使用相同手术方法进行乳腺癌手术,缝合伤口后,对试验组患者使用引流管接负压球引流治疗术后感染,而对照组则使用纱布填塞引流进行治疗,对比两组患者治疗效果。结果两组患者乳腺癌术后皮瓣下积液伴感染均治愈,治愈率均达100.0%,但是两种治疗方法存在明显差异,引流管接负压球引流方法的治愈时间约为5d,而纱布填塞引流约为12d;在治疗过程中,前者换药次数为4次,后者为18次。结论引流管接负压球引流和纱布填塞引流两种方式处理乳腺癌术后皮瓣下积液伴感染均有很好的治疗效果,引流管接负压球引流比纱布填塞引流要节约治疗时间、换药次数更少、费用更少。

OBJECTIVE To compare effects of the drainage with drainage tube connected to a negative pressure ball and the gauze packing drainage for infraskin-flap hydrops accompanied by infection after breast cancer operation , so as to provide a theoretical basis for clinical diagnosis and treatment .METHODS Totally 30 cases were selected of infraskin-flap hydrops accompanied by infection after breast cancer operation who were treated in the hospital during Mar .2011 to Mar .2014 . These patients were randomly divided into the experimental group and the control group with 15 patients in each group .Both groups received the same operation for breast cancer .After suturing operation incisions ,the experimental group received drainage by drainage tube connected to a negative pressure ball for postoperative infection ,while the control group received gauze packing drainage for treatment . The treatment effects were compared between the two groups .RESULTS Both groups were cured with the cure rate up to 100 .0% ,however ,the two treatments had significant differences .The healing time was approximately five days by drainage tube connected to a negative pressure ball and approximately 12 days by gauze packing drainage .The number of dressing was four times by the former treatment and 18 times by the latter treatment . CONCLUSION Both treatments have very good therapeutic effects ,but the drainage by drainage tube connected to a negative pressure ball is time-saving with less dressing times and less cost compared to gauze packing drainage .
