CTA、TCD诊断颅内血管狭窄的准确性对比观察

Paired observation of accuracy difference between CTA and TCD in diagnosis of intracranial vascular stenosis

目的比较CT血管造影(CAT)和经颅多普勒超声(TCD)诊断颅内血管狭窄的准确性。方法选取200例可疑颅内血管狭窄的患者,发病3d内分别进行CTA和TCD检查,比较2种方法诊断颅内血管狭窄的准确性。结果 CAT诊断颅内血管狭窄的灵敏度为68.4%,特异度为65.4%,准确度为68.0%,TCD诊断颅内血管狭窄的灵敏度为52.9%,特异度为57.7%,准确度为53.5%,CAT诊断的灵敏度及准确度均高于TCD,差异均有统计学意义(P<0.05),CAT诊断的特异度高于TCD,但差异无统计学意义(P>0.05)。结论 CAT在诊断颅内血管狭窄中相对TCD具有较高的灵敏度和准确度,具有较高的诊断价值,在临床中值得推广应用。

Objective To compare the accuracy difference between CTA and TCD in diagnosis of intracranial vascular stenosis.Methods 200 patients with suspected intracranial vascular stenosis were selected .All the research object got exami-nation of CTA and TCD within 3 days after attack,and compared the diagnostic accuracy of the two methods .Results The sensitivity of CAT in the diagnosis of intracranial vascular stenosis was 68.4%,the specificity was 65.4%,the accuracy was 68.0%.The sensitivity of TCD in the diagnosis of intracranial vascular stenosis was 52.9%,the specificity was 57.7%,accu-rate rate was 53.5%.The sensitivity and accuracy of CAT were higher than that of TCD ,the differences were statistically sig-nificant（P〈0.05）.The specificity of CAT was higher than TCD,but the difference was not statistically significant （P 〉0.05）.Conclusion Compared with TCD,CAT has a high sensitivity and accuracy in the diagnosis of intracranial vascular stenosis,which has high diagnostic value ,and it is worth in clinical application .
