尿激酶治疗血栓栓塞性疾病的效果研究

Clinical effects of urokinase in the treatment of thromboembolic disease

目的研究尿激酶治疗血栓栓塞性疾病的临床效果。方法选取湛江市第二中医医院2012年6月—2014年6月收治的195例血栓栓塞性疾病患者,根据治疗方法不同分为两组,111例采用尿激酶治疗的患者设为尿激酶组,84例采用低分子肝素治疗的患者设为肝素组,比较两组效果和不良反应。结果尿激酶组肺栓塞(PE)患者总有效率为78.44%,深静脉血栓形成(DVT)患者总有效率为96.67%;肝素组PE患者总有效率为60.98%,DVT患者总有效率为81.40%。两组比较差异有统计学意义(P<0.05)。治疗后2周尿激酶组膝下15cm和膝上15cm健、患肢周径差均小于肝素组(P<0.05)。尿激酶组不良反应发生率为5.41%,肝素组为2.38%,无黏膜或其他脏器出血病例,两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论尿激酶溶栓可提高血栓栓塞性疾病患者的疗效,且不良反应少。

Objective To study clinical effects of urokinase in the treatment of thromboembolic disease. Methods In the Second Chinese Medicine Hospital of Zhanjiang from June 2012 to June 2014,a total of 195 patients with thromboembolic disease were selected,they were divided into two groups depending on the treatments,111 patients treated with urokinase were served as the urokinase group,84 patients treated with low molecular weight heparin were served as the heparin group. Clinical effects and adverse reactions between the two groups were compared. Results In urokinase group,the total effective rate of patients with pulmonary embolism（PE）was 78. 43%,that of patients with deep vein thrombosis（DVT）was 96. 67%;in heparin group,the total effective rate of patients with PE was 81. 4%,that of patients with DVT was 60. 98%,there were significant differences between the two groups（P 〈0. 05）. Two weeks after treatment,healthy and limb circumference over knee 15cm of urokinase group were less than those of heparin group（P 〈0. 05）. Adverse reactions rate of urokinase group was 5. 41%,that of heparin group was 2. 38%,no one occurred mucosal or other organ bleeding,and the adverse reactions rate between the two groups was not statistically significant（P 〉0. 05）. Conclusion Urokinase thrombolytic therapy can improve the efficacy of patients with thromboembolic disease,and has fewer adverse reactions.
