摘要
目的:探讨红外热成像技术在腰椎间盘突出症证候疗效评定中的应用价值。方法:对60例腰椎间盘突出症患者采用非手术综合疗法治疗,分别于治疗前后测量双侧大腿后侧、大腿后外侧、小腿后侧、小腿后外侧和足底的体表温度,并于健侧和患侧之间进行比较。对60例患者的临床疗效进行评定,分析临床疗效与红外热成像图温差的关系,并对红外热像图温差值与证候积分差值进行相关性分析。结果:1健侧与患侧下肢温度的比较。60例患者大腿后侧、大腿后外侧、小腿后侧、小腿后外侧及足底部的健、患侧温度分别为(34.46±1.08)℃、(31.87±1.10)℃,(34.45±1.07)℃、(31.79±1.07)℃,(33.43±1.05)℃、(31.1±1.06)℃;(32.65±1.08)℃、(30.20±1.06)℃,(30.75±1.08)℃、(28.36±1.07)℃。患侧温度明显低于健侧(P<0.05)。2临床疗效与红外热成像图温差的关系。60例患者临床控制8例、显效28例、有效24例。红外热成像显示,不同临床疗效下的患肢温差变化之间的差异有统计学意义(P<0.05);临床疗效好的患者,其患肢温差变化大。3红外热像图温差值与证候积分差值的相关性分析。治疗前后红外热像图温差值与治疗前后证候积分差值呈正相关(r=0.584,P=0.001)。结论:红外热成像技术在腰椎间盘突出症的证候疗效评定中具有重要的应用价值。
Objective: To explore the applied value of infrared thermal imaging technology in curative effect evaluation of SYMPTOM COMPLEX for patients with lumbar disc herniation( LDH). Methods: Sixty patients with LDH were treated with non-operative combined therapy. The shell temperatures of the back of thigh,posterolateral thigh,the back of leg,posterolateral leg and planta pedis were measured before and after treatment respectively,and were compared between healthy side and affected side. Then the clinical effects were evaluated for 60 patients,and the relationship between clinical effects and temperature changes of infrared thermal imaging were analyzed. Meanwhile,correlation analysis were applied to study the relationship between temperature change of infrared thermal imaging and change of SYMPTOM COMPLEX scores. Results: The temperatures of the back of thigh,posterolateral thigh,the back of leg,posterolateral leg and planta pedis of uninjured side and affected side were 34. 46 + /- 1. 08,31. 87 + /- 1. 10,34. 45 + /- 1. 07,31. 79 + /- 1. 07,33. 43 + /- 1. 05,31. 1 + /-1. 06,32. 65 + /- 1. 08,30. 20 + /- 1. 06,30. 75 + /- 1. 08,28. 36 + /- 1. 07 degrees centigrade respectively. The temperatures of affected side were obviously lower than that of healthy side( P 〈0. 05). Eight patients got an excellent result,28 good,and 24 fair. The infrared thermal imaging showed that there was statistical difference in the temperature changes of affected limbs between different clinical effects( P 〈0. 05),and big temperature changes of affected limbs were found in patients who got excellent or good clinical result. The results of correlation analysis showed that the temperature changes of infrared thermal imaging were positively correlated with the changes of SYMPTOM COMPLEX scores after the treatment( r = 0. 584,P = 0. 001). Conclusion: The infrared thermal imaging technology has important applied value in curative effect evaluation of SYMPTOM COMPLEX for patients with LDH.
