糖尿病膝下动脉闭塞行膝下动脉成形术并发症的处理和预防

Treatment and Prevention of Diabetic Complication Following Foot Knee Artery Figuration

目的探讨糖尿病膝下动脉闭塞行膝下动脉成形术并发症的处理方法和预防措施。方法回顾分析糖尿病膝下动脉闭塞行膝下动脉成形术169例的临床资料,总结并发症发生原因、处理方法和预防措施。结果本组169例发生手术并发症29例,发生率17.2%,其中动脉穿孔12例,动脉内膜撕裂夹层7例,动脉痉挛5例,导丝断裂2例,胫后动脉栓塞、动静脉瘘、假性动脉瘤各1例,经相应积极处理后均收到满意效果。术后随访6个月,1例因再次出现下肢急性缺血导致截肢,余28例未见异常。结论膝下动脉成形术中注重操作技巧、准确评估病情,可预防或减少并发症发生。

Objective To study the treatment and prevention of Diabetic complications following foot knee artery figu-ration. Methods Clinical data of 169 cases of diabetic foot knee artery figuration undergoing interventional treatment proce-dures were analyzed retrospectively. The causes of complications, preventive and treatment methods were summarized. Results 29 cases of diabetic foot knee artery figuration occurred during 169 times of interventional treatment procedures. The inci-dence rate of complication was 17. 2% . The complications included： artery perforation in 12 cases, aortic dissection in 7,arte-riolar spasm in 5 cases, Guide wire fracture in 2 cases,arterial embolism in 1 case,arteriovenous fistula in 1 case, traumatic pseudoaneurysm in 1 and Amputation in 1 case. All complications were rated excellent after treatment. All the cases were re-viewed within 6 months. During the follow-up period no complications occurred in any patients except one who was amputated because of reocclusion of artery of lower extremity. Conclusion Complications in diabetic foot knee artery figuration can be effectively treated and prevented if correct operation and evaluation are conducted.
