鼻内镜联合局部应用糖皮质激素对鼻息肉患者的临床疗效及手术复发率的影响

The Efficacy and Influence on Surgical Recurrence Rate of Nasal Endoscopy Combined with Topical Corticosteroids Treating Patients with Nasal Polyps

目的探讨鼻内镜联合局部应用糖皮质激素对鼻息肉患者的临床疗效及手术复发率的影响。方法将2010年1月至2012年9月来荆州市中心医院耳鼻喉科就诊的144例鼻息肉患者依据随机数字表法分为两组:对照组72例采用常规内镜手术治疗,试验组72例采用鼻内镜手术联合局部应用糖皮质激素,比较两组患者的临床疗效及手术复发率。结果试验组患者的总有效率显著高于对照组(95.9%比61.2%),差异有统计学意义(P<0.05);试验组患者术中出血量显著少于对照组,头痛、鼻塞、脓涕、嗅觉减退的发生率均低于对照组,差异有统计学意义(P<0.05);试验组半年、1年的手术复发率均显著低于对照组,差异有统计学意义(P<0.05)。结论鼻内镜联合局部应用糖皮质激素治疗鼻息肉患者具有较为理想的临床疗效,且手术复发率较低,十分安全可靠。

Objective To investigate the clinical efficacy of nasal endoscopy combined with local corticosteroid therapy on nasal polyps,and recurrence the rate after surgery. Methods A total of 144 nasal polyps patients admitted to Jingzhou Central Hospital from Jan. 2010 to Sep. 2012 were randomly divided into control group and trial group. The control group was treated with conventional endoscopic surgery,while the trial group was treated with endoscopic surgery combined with local glucocorticoid. The clinical efficacy and surgical recurrence rate were compared between the two groups. Results The total effective rate of the trial group was significantly higher than the control group（ 95. 9% vs 61. 2%）,the difference was statistically significant（ P 0. 05）. Blood loss in the trial group was significantly less than the control group. Headache,nasal congestion,purulent nasal discharge,loss of sense of smell were lower than the control group,the differences were statistically significant（ P 0. 05）. After six months and one year,the recurrence rate of the trial group was significantly lower than the control group,the difference was statistically significant（ P 0. 05）.Conclusion Nasal endoscopy combined with local corticosteroid treatment has a more satisfactory clinical effect and lower recurrence rate,which is safe and reliable,worthy of clinical application.
