多层螺旋CT与X线平片诊断早期周围型肺癌的临床对比研究 被引量:31

摘要 目的探讨多层螺旋CT与X线平片对早期周围型肺癌诊断的临床应用。方法选取2011年3月至2013年4月梁平县人民医院收治的早期周围型肺癌患者68例,对其进行CT和X线扫描。统计两种扫描方法观察到的肺内、肺外病灶数量,并统计两种检查方法的阳性率。结果螺旋CT检出肺内病灶、肺外病灶以及总病灶数分别为197、82、279个,X线分别为119、40和159个,螺旋CT检出数量明显多于X线检查。螺旋CT检测肺内病灶和肺外病灶的阳性率分别为91.2%和48.5%,X线分别为83.8%和38.2%,螺旋CT肺内和肺外病灶阳性率均显著高于X线检测,差异有统计学意义(P<0.01)。结论通过X线和多层螺旋CT扫描可以对大多数早期周围型肺癌进行诊断,且CT检查的阳性率明显高于X线。在临床中选择何种检查方法,应根据具体情况合理选择,必要时也可联合检查,定期随访。 Objective To investigate the clinical peripheral lung cancer diagnosis. Methods A total application of multi-slice CT and X-ray in early-stage of 68 cases with early-stage peripheral lung cancer admitted to Liangping County People's Hospital from Mar. 2011 to Apr. 2013 were included in the study,all of which received X-ray and CT scan. The observed number of intrapulmonary and extrapulmonary lesions by the two methods were recorded, and the positive rates of the two methods were statistically processed. Results The spiral CT detected intropulmonary, extrapulmonary and total lesions were 197,82,279, while X-ray detected numbers were 119,40 and 159 respectively, the spiral CT detected numbers significantly more than the that of the X-ray detected.. The positive rate of spiral CT detected intrapulmonary and extrapulmonary lesions was 91.2% and 48.5% respectively,of X-ray was 83.8% and 38.2% respectively,the spiral CT's numbers .were significantly higher than the X-ray' s, with statistically significant differences ( P 〈 0.01 ). Conclusion X-ray and multislice spiral CT scans can diagnose most of the early-stage stage peripheral lung cancer, and the sensitivity of CT is significantly higher than X-ray. The choice in clinical examination should be a reasonable choice depending on the specific situation, and joint inspection and regular follow-up can be adopted if necessary.
