甘露醇与甘油果糖联合治疗脑出血的疗效及其对肾功能损害的临床观察研究 被引量:13

The Efficacy of Mannitol Associate with Glycerol and Fructose in the Treatment of Cerebral Hemorrhage and Its Impairment to Kidney

摘要 目的观察甘露醇与甘油果糖联合治疗脑出血患者的疗效及其对肾功能损害的情况。方法选取2012年2月至2013年2月在迁西县人民医院住院的82例脑出血患者为研究对象,采用随机数字表法分为两组:观察组(41例)采用甘露醇与甘油果糖交互脱水治疗;对照组(41例)采用单纯甘露醇脱水治疗。观察两组患者的临床疗效、神经功能评分、CT情况,以及肾功能损害、电解质紊乱发生情况。结果治疗后,观察组治疗总有效率为95.1%,高于对照组的78.0%,两组间差异有统计学意义(P<0.05);观察组患者的神经功能评分、水肿范围和血肿范围均小于对照组,差异有统计学意义(P<0.05);观察组肾功能损害及电解质紊乱发生率与对照组比较,差异无统计学意义(P>0.05)。结论甘露醇与甘油果糖交替治疗脑出血患者的治疗效果较佳,可有效改善神经功能,而且安全性较高。 Objective To observe the effect of mannitol associate with glycerol and fructose treatment for the patients with cerebral hemorrhage and the damage to the kidney .Methods A total of 82 inpatients of cere-bral edema complicate cerebral hemorrhage treated in Hebei province Tangshan City Qianxi County People′s Hospital from Feb.2012 to Feb.2013 were included as research objects.They were randomly divided into mannitol and glycerol fructose alternate dehydration treatment group(observation group for short) and the sim-ple mannitol dehydration treatment control group ( control group for short) according to random number-table method, 41 cases in each group.The clinical effect,neurological function score,CT results,impairment of renal function and the electrolyte disturbance of the two groups were observed.Results After treatment, the effec-tive rate of the observation group was 95.1%,4higher than 78.0%of the control group with a statistically sig-nificant difference (P〈0.05).All the indexes of the observation group were more obviously decreased than the control group with statistically significant differences(P〈0.05).The incidence of renal damage and elec-trolyte disturbances of the observation group were lower than the control group ,but the differences were not sta-tistically significant(P〉0.05).Conclusion Alternate application of mannitol and glycerol fructose has a good therapeutic effect on cerebral hemorrhage, which not only can effectively improve the neural function, but also has high safety.
