关节镜下微骨折术治疗膝关节软骨损伤的临床疗效观察 被引量:30

Clinical efficacy of arthroscopic microfracture surgery on knee cartilage injury

摘要 目的:探讨关节镜下微骨折术在膝关节软骨损伤治疗中的临床疗效。方法将76例研究对象随机分为治疗组(TG,n=38)与对照组(CG,n=38),TG行关节镜下微骨折术,CG行关节清理术,术后随访6~18个月,比较两种不同治疗方法的临床疗效。结果 TG患者HSS评分(86.36±4.17)分,CG患者HSS评分(63.71±6.20)分;两组比较差异有统计学意义(t=35.785, P=0.000)。TG中HSS评分>85分、60~85分、<60分分别为15例、19例及4例;而CG分别为7例、13例和18例;两组比较差异有统计学意义(χ^2=6.258,P=0.013)。TG显效14例,有效20例,无效4例,总有效率为89.47%,与CG总有效率为50.00%相比,差异有统计学意义(χ^2=8.130,P=0.000)。TG中仅3例出现轻微疼痛,CG中10例出现关节肿胀;两组比较,TG明显低于CG(χ^2=4.547,P=0.033)。结论关节镜下微骨折术具有、术后并发症少等优点,对于膝关节软骨缺损患者关节功能的改善有积极的促进作用。 ObjectiveThis study aims to investigate the clinical efficacy of arthroscopic microfracture surgery on the knee cartilage injury.Methods76 patients were randomly divided into treatment group (TG,n=38) and control group (CG,n=38). TG were performed arthroscopic microfracture surgery, and CG was performed joint debridement. All the patients were had a 6-18 months follow-up. ResultsThe HSS score was significant higher of 86.36±4.17 in TG than that of 63.71±6.20 (t=35.785, P=0.001). The number of patients whose HSS score〉85, 60-85, 〈60 were 15, 19 and 4 in TG respectively, and were 7, 13 and 18 in CG respectively; there was significant difference between two groups (χ^2=6.258, P=0.013). 14 cases were in a significant effect, 20 cases were in effective, 4 cases were in invalid, and the total effective rate was 89.47%, which was statistically significant better than CG (50.00%)(χ^2=8.130, P=0.000). Only 3 cases had slight pain in TG, which were significantly lower than that of 10 cases in CG (χ^2=4.547,P=0.033).ConclusionArthroscopic microfracture surgery has the advantages of reliable curative effect, simple operation, less postoperative complications. It can improve the function of knee joint function of patients with articular cartilage defects.
