健肝乐颗粒联合拉米夫定治疗HBeAg阳性慢性乙型肝炎疗效分析

Study on therapeutic effect of combined Jianganle granules with lamivudine in treatment of patients with HBeAg positive chronic hepatitis B

目的研究健肝乐颗粒联合拉米夫定治疗HBeAg阳性慢性乙型肝炎的临床疗效。方法选择HBeAg阳性慢性乙型肝炎148例患者的临床资料,按照治疗完成时间分为正常治疗组患者88例和延长治疗组60例。两组患者均联合使用健肝乐颗粒、拉米夫定连续治疗12个月,正常治疗组治疗停止,延长治疗组继续进行治疗方法治疗4个月。观察两组患者治疗12个月以及治疗完成后随访6个月时肝功能,乙型肝炎DNA、HBsAg、抗-HBs等指标定量检测,抗-HBe、HBeAg等指标半定量检测结果。结果两组患者年龄、HBeAg、丙氨酸氨基转移酶(ALT)、乙肝DNA对应比较,差异均无统计学意义(P均>0.05)。治疗完成后以及治疗后随访6个月,两组患者HBeAg、HBsAg、ALT、乙肝病毒DNA均有一定的改善,但延长治疗组各项指标改善均优于正常治疗组,且两组患者治疗完成后以及治疗后随访6个月HBeAg转阴率、HBsAg转阴率分别比较,差异均有统计学意义(P<0.05)。结论健肝乐颗粒联合拉米夫定治疗HBeAg阳性慢性乙型肝炎疗效显著,且治疗时间越长,疗效越显著。但仍需进一步研究确定。

Objective To study the clinical curative effect of combined Jianganle granules with lamivudine in treatment of patients with HBeAg positive chronic hepatitis B,and to provide the experience in clinical treatment of patients with chronic hepatitis B. Methods The clinical data of 148 patients with HBeAg positive chronic hepatitis B were selected for this study,and they were divided into regular treatment group（ n=88 cases）and treatment extended group（ n=60 cases）. according to the completion of treatment course. Patients in both 2 groups were adminis-trated with Jianganle granules and lamivudine for 12 months,and the therapeutic course of patients in prolonged treatment group was extended for 4 months. The efficacy of treatment was observed in these two groups at 12 months after the completion of treatment course and 6 months follow-up period,the indexes of liver function,hepatitis B DNA,HBsAg,anti-HBs and other indicators for quantitative detection,and the results of semi- quantitative-HBe,anti HBeAg indexes were compared between these 2 groups. Results In comparison with two groups of patients with age,HBeAg,ALT,hepatitis B DNA correspondence,there was no significant statistical difference（ P ﹥0. 05）. After completion of treatment course,patients in 2 groups were followed up for 6 months,the improvement in HBeAg,HBsAg,ALT,hepatitis B DNA in patients of 2 groups was prolonged,but the indexes of extended group were better than those of normal treatment group,and patients in two groups after completion of therapy were followed up for 6 months. There was significant difference in HBeAg inversion rate and HBsAg seroconversion rate respectively（ P ﹤0. 05）. Conclusion The efficacy of combined Jianganle granules with lamivudine in treatment of patients with HBeAg positive chronic hepatitis B is significant,but further study is needed.
