摘要
目的探讨Toll样受体4(TLR4)在儿童哮喘急性发作中的作用及可能机制。方法选择2012年1月至2014年6月期间40例哮喘急性发作患儿为研究对象。选择36例哮喘缓解期患儿为疾病对照组。选择40例健康儿童为健康对照组。检测受试者TLR4、白细胞介素6(IL-6)、白细胞介素8(IL-8)、肿瘤坏死因子α(TNF-α)的表达情况。结果哮喘急性发作组患儿外周血单核细胞(PBMCs)中,TLR4 mRNA PCR检测含量为(0.032±0.009),哮喘缓解组和健康对照组TLR4表达量分别为(0.015±0.007)和(0.012±0.005),急性发作组TLR4表达量高于缓解组和对照组,差异有统计学意义(F=89.030,P=0.000),哮喘缓解组和对照组TLR4表达量差异无统计学意义(P>0.05);急性发作组IL-6、IL-8、TNF-α的水平高于对照组,差异有统计学意义(P<0.01);TLR4阻断后急性发作组IL-6、IL-8、TNF-α的水平明显低于TLR4阻断前,差异有统计学意义(P<0.01);TLR2阻断前、阻断后急性发作组PBMCs上清中IL-6、IL-8、TNF-α的水平无显著差异(P>0.05)。结论 TLR4活性增强可导致儿童哮喘的急性发作,其可能机制之一是促进IL-6、IL-8、TNF-α等指标的表达进而引起哮喘急性发作。
Objective To study the role of toll-like receptor 4 in childhood asthma and its possible mechanism. Methods Between January 2012 and June 2014,40 cases of children with acute attack of asthma from our hospital were selected as the research object. 36 cases of children with remission of asthma were selected as disease control. 40 cases of healthy children were selected as healthy controls. The expression of TLR4,IL-6,IL-8,TNF alpha of all subjects were tested. Results The PBMCs by TLR4 mRNA PCR test in children with acute attack of asthma was( 0. 032 ± 0. 009). And those of relieving asthma group and healthy controls TLR4 expression quantity were( 0. 015 ± 0. 007) and( 0. 012± 0. 005) respectively. The TLR4 expression quantity of acute attack of asthma was higher than those of ease group and the control group. The difference was statistically significant( F = 89. 030,P = 0. 000). The TLR4 expression difference of ease asthma group and the control group had no statistical significance( P 0. 05). IL-8,IL-6,TNF alpha of acute attack of asthma were higher than the control group,and the difference was statistically significant( P 0. 01). The levels of IL-8,IL-6,TNF alpha in acute attack of asthma after TLR4 blocked were obviously lower than that before the TLR4 blocked,and the difference was statistically significant( P 0. 01). Before and after TLR2 blocked in the acute attack of asthma group PBMCs' levels of IL-8,IL-6,TNF alpha were of no significant difference( P 0. 05). Conclusion High expression of TLR4 can lead to childhood with acute attack of asthma. One of its possible mechanism is promoting the expression of IL-8,IL-6,TNF alpha,and then leading to acute attacks of asthma.
