超声心动图指标与心房颤动患者血栓栓塞风险的相关性研究 被引量:3

摘要 目的探讨超声心动图指标与CHADS2和CHA2DS2-VASc评分评价心房颤动血栓栓塞风险的相关性。方法选取于2012年6月至2014年6月收集的300例心房颤动的患者,按照CHADS2与CHA2DS2-VASc评分标准将所有患者分为三组,分别为低危组、中危组与高危组。对所有患者进行超声心动图检查,对各指标与心房颤动血栓栓塞风险计分的相关性进行分析。结果通过CHADS2评分系统评分,低、中、高危组患者分别为73例、72例与155例。与CHADS2评分存在一定相关性的超声指标主要有室间隔厚度、左室后壁厚度、左房内径、肺动脉内径、右室内径、主动脉内经以及LVEF。通过CHA2DS2-VASc评分,低、中、高危组患者分别为32例、66例与202例。与CHA2DS2-VASc评分具有显著相关性的超声指标主要有室间隔厚度、左室后壁厚度、左房内径、肺动脉内经、主动脉瓣瓣口流速以及LVEF。结论随着CHADS2与CHA2DS2-VASc评分分数的提高,超声心动图指标主要表现为左房扩大、左室肥厚、肺动脉内经增大以及LVEF降低。 Objective To explore the relationship between the echocardiographic indices with the risk score of atrial fibrillation thromboembolism. Methods There are 300 cases of patients with atrial fibrillation come to our hospital for treatment at the time from June 2012 to June2014. These patients were divided into low risk group,the moderate and high risk group according to the scoring criteria of CHADS2 and CHA2DS2-VASc. The supersonic cardiogram and the echocardiographic indices of all the patients were checked. The relationship between the several indicators with the risk score of the atrial fibrillation thromboembolism were analyzed. Results Through the scoring system of CHADS2,we divided the patients into low risk group,the moderate and high risk group with 73 patients,72 patients and 155 patients respectively. There are several supersonic indicators associated with the score of CHADS2,such as the thickness of ventricular septal,thickness of left ventricular posterior wall,diameter of left atrial,diameter of pulmonary artery,diameter of right ventricular,diameter of aortic and LVEF. Through the score of CHA2DS2-VASc,we can find out the number of patients had been seperated into low risk group,the moderate and high risk group is 32,66 and202 respectively. There are also have several indicators having significant relationship with the score of CHA2DS2-VASc,such as the thickness of ventricular septal,thickness of left ventricular posterior wall,diameter of left atrial,diameter of pulmonary artery,velocity of aortic leaf and LVEF. Conclusio With the improvement of scores of CHADS2 and CHA2DS2-VASc,the indices of echocardiographic included mainly for the following several aspects such as the expand of left atrial,left ventricular hypertrophy,increasing of the diameter of pulmonary artery and reducing of LVEF.
