基于舌、面、脉诊图像参数的冠心病中医疗效评价研究

Evaluation of curative effect of treating coronary heart disease with traditional Chinese medicine based on image parameters of tongue, face and pulse

目的:观察冠心病患者中医治疗前后舌、面、脉诊图像参数的变化特征,寻找舌、面、脉诊图像参数的变化规律,客观评估舌、面、脉诊图像信息在冠心病中医疗效评价体系中的有效性和实用性,为中医疗效评价方法提供客观化指标。方法:运用上海中医药大学自行研制的中医舌诊、面诊数字化检测仪以及Z-box脉诊仪采集冠心病患者中医治疗前后的舌、面、脉诊图像,运用配对t检验的统计学方法,寻找患者舌、面、脉诊图像参数的变化特征及规律。结果:1治疗前后患者的舌诊图像参数R、G、B、H、S、V值无显著性差异,但治疗后的R、G、B、H、V值呈现下降趋势;2治疗后患者的面诊图像参数H值较治疗前降低(P<0.05),其余治疗前后的面诊图像参数R、G、B、S、V值无显著性差异,但治疗后的R、G、S、V值呈现下降趋势,B值呈现上升趋势;3治疗前后患者的脉诊图像参数h1、h3、h4、h5、t、w、h3/h1、h4/h1、h5/h1、w/t值无显著性差异,但治疗后的h4、h5、t、h5/h1值呈现明显上升趋势,h1、w、h3、h3/h1、w/t值呈现下降趋势。结论:舌、面、脉诊图像参数可以作为中医临床疗效评价的新指标,同时为建立具有中医特色的疗效评价体系提供了新的思路和方法。

Objective： Look for the change rule of tongue, face and pulse image parameters to objectively evaluate the effectiveness and practicability of tongue, face and pulse image parameters in TCM evaluation system of coronary heart disease （CHD）, in order to provide objective index for TCM efficacy evaluation method by observing the change features of tongue, face and pulse image parameters which collected from CHD patients. Methods： The tongue, face and pulse images of CHD patients were collected by digital detector of tongue and face diagnosis and pulse meter developed by Shanghai University of TCM, and looked for the change rules and features by using statistical method of roaching t test. Results： ①There was no significant difference in R, G, B, H, S, V value of CHD patients between pretherapy and post-treatment, but after treatment, the R, G, B, H, V value showed a downward trend. ②The H value of face image parameters of CHD patients after treatment was significant lower than before treatment （P〈0.05）, while the difference in value of R, G, B, S, V between pretherapy and post-treatment was not significant. R, G, S, V value showed a downtrend after treatment, and B value showed an uptrend. ③There was no significant difference in hl, h3, h4, fiS, t, w, h3/hl, h4/hl, hS/hl, w/t value of pulse image parameters between pretherapy and post-treatment, while the h4, hS, t and h5/hl value showed an uptrend after treatment, and the hl, w, h3, h3/hl, w/t value showed and dawntrend Conclusion： Tongue, face and pulse image parameters could be as new indexes for evaluation of clinical curative effect of traditional Chinese medicine, and it also provides a new idea and method for the establishment of efficacy evaluation system with Chinese medicine characteristic.
