乳腺癌分子亚型对新辅助化疗疗效及预后的预测价值 被引量:18

摘要 目的:探讨乳腺癌分子亚型对新辅助化疗(TE方案)疗效及预后的预测价值。方法:回顾性分析195例接受TE方案化疗的乳腺癌患者。依据雌激素受体、孕激素受体和人表皮生长因子受体2表达水平(免疫组化法)将患者分为Luminal A型、Luminal B型、HER 2+型和Basal-like型。观察患者的临床特点、疗效及预后。结果:basal-like、HER 2+型的临床完全缓解率分别为44.8%、50.0%,显著高于Luminal A型(15.2%)和Luminal B型(22.2%),P<0.05;basal-like、HER2+型的病理完全缓解率分别为31.0%、27.8%,显著高于Luminal A型(4.5%)和Luminal B型(5.6%)(P<0.05);basal-like与HER 2+型的3年无瘤生存率(分别为77.8%、82.8%)、3年总生存率(83.4%、89.6%)显著低于Luminal A和B型的无瘤生存率(92.0%,91.6%)和总生存率(97.3%,94.4%)(P<0.05)。结论:分子分型可作为TE方案治疗乳腺癌疗效和预后的预测因子。basal-like及HER 2+型对化疗更敏感,但3年生存率较差。
