深圳结核病住院患者营养风险筛查及其对临床结局和生活质量的评估 被引量:20

摘要 目的:对深圳地区结核病住院患者进行营养风险筛查,同时评估营养风险与并发症的发生率和生活质量的关系。方法:从深圳市第三人民医院结核科、外科连续定点选择1 374例结核住院患者,收集营养风险筛查、并发症和生活质量等数据。结果:入院时结核病住院总患者、年轻患者、老年患者的营养风险发生率分别为77.7%、70.9%、88.9%。结核病住院总患者、年轻患者、老年患者从入院到入院2周后营养风险的发生率均显著增加(P<0.05)。入院时、入院2周或出院时老年患者比年轻患者营养风险的发生率均显著增加(P<0.05)。关于生活质量(SF-16)的分数,生理机能、生理职能、躯体疼痛、疲劳和一般健康状况均显著低于有营养风险的结核病患者。结论:大量的结核病住院患者存在营养风险,并且往往在住院期间加重,这可能导致并发症的发生率增加和生活质量的下降。 Objective To conduct nutritional risk screening and evaluate the relationship of nutritional risk to complication rate and quality of life in tuberculosis inpatients of Shenzhen. Methods A total of 1 374 patients from tuberculosis department and surgery department in the third people′ s hospital of Shenzhen were consecutively enrolled. Data were collected on the nutritional risk screening, complication and quality of life.Results The occurence of nutritional risk at admission of inpatients with tuberculosis among total, younger,elderly patients was 77.7%, 70.9%, 88.9%, respectively. There was a significant increase in the occurrence from admission to 2 weeks in all patients(P 0.05). On admission and 2 weeks after admission or discharge,the occurence in those patients ≥ 65 years was significantly higher than that in the younger ones(P 0.05) On the scales of quality of life(SF-36), the scores of physical functioning, role-physical, bodily pain, fatigue and general health were significantly lower than in the patients at risk. Conclusion A large proportion of inpatients of tuberculosis is at nutritional risk and tended to be worsen during the course of admission, which has associated with increased complication rate and lower scores of quality of life.
