直肠癌低位保肛临床应用

To Explore the Clinical Application of Anus Preservation Operation for Low Rectal Cancer

目的探讨并分析直肠癌低位保肛临床应用的效果。方法回顾性分析2008年1月-2014年1月到我院实施治疗的79例直肠癌患者临床资料,其中42例低位直肠癌者中23例行保肛治疗,观察分析直肠癌低位保肛治疗情况。结果 23例患者顺利完成手术,2例因术中出血和直肠前壁受损改施Miles术。术后3例出现吻合口瘘,2例出现吻合口狭窄,六个月完成治疗,随访期间23例患者控便功能保持良好。结论对低位直肠癌实施保肛治疗,严格按照手术要求和规范执行,可获得良好效果,治疗期间注意盆腔自主神经和直肠全系膜切除保护,预防吻合口瘘和狭窄问题的发生。

Objective To study and analyze the effect and clinical application of the anus preservation operation in low rectal carcinoma. Methods 79 cases of colorectal cancer patients in our hospital from January 2008 to January 2014 were selected. Their clinical data were retrospective analyzed. There were 42 cases of low rectal cancer in which 23 cases of patients received anal sphincter preservation operation. The conditions of the treatment were observed and analyzed. Results The operation of 23 patients were successfully completed. 2 cases received the modified application of Miles operation because of intraoperative bleeding and the damage anterior wall of the rectum. After the treatment, 3 cases had anastomotic fistula, 2 cases had anastomotic stenosis. Six months after the treatment, 23 patients received follow-up visits. They maintained good control and function. Conclusion The implementation of anus preservation operation for low rectal cancer, with strict accordance of the operation requirement and standard execution, can obtain good effect. During the treatment period, we should pay attention to the protection of pelvic autonomic nerve and total mesorectal excision and the prevention of the problem of anastomotic leakage and stenosis.
