摘要
目的探讨糖尿病患者尿路感染的病原菌分布及耐药性。方法选取660例2型糖尿病患者作为研究对象。收集患者清洁中段晨尿,经培养、分离、鉴定等处理,通过K-B法检测药敏试验。结果 660例2型糖尿病患者中,150例并发尿路感染(22.7%),检出137株病原菌(91.3%),其中革兰阴性菌63株(构成比为46.0%,其中包括46株大肠埃希菌)、革兰阳性菌34株(24.8%,其中包括14株粪肠球菌)、真菌40株(29.2%,其中包括29株白色假丝酵母菌)。大肠埃希菌对亚胺培南敏感、白色假丝酵母菌对制菌霉素、两性霉素B敏感,粪肠球菌对环丙沙星、氧氟沙星耐药率为100%,病原菌呈多重耐药性。结论 2型糖尿病患者尿路感染的病原菌呈多样性,耐药性呈上升趋势,并且存在多重耐药现象,应积极做好尿液培养和药敏试验,选择敏感抗生素,从而指导临床治疗,同时,有效控制高血糖水平,避免严重并发症的发生。
Objective To study the distribution and drug resistance of pathogenic bacteria in patients of diabetes mellitus combined with urinary tract infection .Methods The 660 cases of type 2 diabetes mellitus were regarded as the investigation subjects .The urine subjects were collected and then were received the processing of culture ,isolation ,identification .The drug sensitivity was detected by the K‐B test .Results The 150 cases combined with urinary tract infection (22.7% ) in the 660 cases of type 2 diabetes mellitus .The 137 strains of pathogenic bacteria (91.3% ) were detected and included the 63 strains of gram negative bacteria (the ratio was 46 .0% ,including 46 strains of Escherichia coli) ,34 strains of gram positive bacteria (the ratio was 24.8% ,including 14 strains of Enterococcus faecalis) ,40 strains of fungi (the ratio was 29 .2% ,including 29 strains of Candida albicans) .The Escherichia coli was sensitive to imipenem ,and the Candida albicans was sensitive to nystatin and am‐photericin B ,the rate of drug resistance of Enterococcus faecalis to ciprofloxacin and osloxacin were 100 .0% .The pathogenic bacteria were observed the multi drug resistance .Conclusion The pathogenic bacteria for patients of type 2 diabetes mellitus combined with urinary tract infection is diversity with the rising rate of drug resistance ,and the existence of multiple drug re‐sistance .The good job of urine culture and drug sensitive test should done and then choose the sensitive antibiotics to guide the clinical treatment .As the same time ,need effectively control the high blood sugar levels and avoid the occurrence of serious complications .
