The Study of Reproductive Hormones Associated with Body Mass Index in Elderly Women with Stress Urinary Incontinence

摘要目的观察老年女性压力性尿失禁(SUI)患者生殖相关激素与体重指数的改变,分析相关性并探讨可能机制。方法 79例SUI患者及18例非SUI患者纳入研究。根据尿失禁程度分为1级(n=27)、2级(n=34)、3级(n=18)。记录所有患者的体重指数(BMI)并检测生殖相关激素水平(雌二醇、孕酮、睾酮、卵泡刺激素、黄体生成素),分析两者的相关性。结果 3级SUI患者较1、2级患者体重指数明显增高(P<0.05),而血清雌二醇(E2)反而降低(P<0.05),其余生殖相关激素无明显改变(P>0.05)。相关性分析提示:所有SUI患者体重指数与E2呈负相关(r=-0.617,P<0.05)。亚组分析,两者在SUI 1、2、3级的相关值分别为-0.578,-0.624,-0.648(P<0.05)。结论体重指数越高,SUI越严重,而E2越低,其余生殖相关激素变化不大;BMI与E2呈显著负相关;两者相互作用又共同加重SUI。 Objective To observe the changes of reproductive hormones levels and body mass index （BMI） in older women with stress urinary incontinence （ SUI）, analyse the correlation and explore the possible mechanism. Methods 79 SUI patients and 18 non-SUI patients were enrolled in this study. According to the se- verity of disease, the SUI patients were divided into three groups ： grade 1 group （ n = 27 ）, grade 2 group （ n = 34） ,grade 3 group （n = 18）. The BMI was recorded and the serum reproductive hormones levels including es- tradiol, progesterone, testosterone, follicle stimulating and luteinizing hormone were measured. The correlation of both indics was analyzed. Results Compared to the grade 1 and 2 SUI patients, grade 3 SUI patients showed higher BMI （P 〈 0.05 ） and lower serum estradiol levels （ P 〈 0.05 ）. No changes of the rest hormone levels were found in each group （P 〉 O. 05 ）. The BMI was associated with the levels of estradiol and showed negative correlation （r = -0. 617 ,P 〈0.05）. Subgroup analysis suggested that the correlation value in each group was - 0.578, - 0.624, - 0.648, respectively （ P 〈 0.05 ）. Conclusion The BMI is higher, the SUI is more serious, and the estradiol levels are lower. The remaining reproductive hormones changed little. BMI was negatively cor- related with estradiol, and both of them aggravated the stress urinary incontinence together
