摘要
患者男,29岁。双手近端指间关节梭形肿胀伴皮肤增厚、粗糙5年,无疼痛,双手指活动不受影响。左手部MRI查示左手诸指的近端指间关节周围软组织肿胀。组织病理学检查示:表皮角化过度,棘层肥厚,真皮胶原纤维增多,排列不规则,真皮网状层成纤维细胞轻度增生。其临床表现、MRI及组织病理学检查符合厚皮指症的诊断。
A 29-year-old man presented with 5-year history of bilateral, painless, and fusiform swelling of the proximal phalanges of the hands with the overlying skin thickened and roughened. The finger movement was not affect- ed by swelling. MRI scan of the left hand showed soft tissue swelling. Histopathologic examination demon- strated epidermal hyperplasia with compact orthokeratosis, thickening of the dermis with an increase of colla- gen bundles, and a slight proliferation of fibroblasts in the reticular dermis. These findings are consistent with pachydermodactyly.
