腹腔镜联合孕三烯酮治疗卵巢子宫内膜异位囊肿的临床疗效

Efficacy of laparoscope conservative surgery with gestrinone in treatment of ovarian endometriosis cyst

目的研究腹腔镜联合孕三烯酮治疗卵巢子宫内膜异位囊肿患者血清高迁移率组蛋白B1(HMGB1)、视黄醇结合蛋白4(RBP4)、基质金属蛋白酶-1(MMP-1)、组织抑制因子-1(TIMP-1)的影响及临床意义。方法选取2012年1月—2013年12月就诊于浙江省人民医院门诊并住院进行治疗子宫内膜异位囊肿患者68例为实验组;同期于医院体检正常育龄妇女70例为对照组。应用酶联免疫法检测2组血清血清高迁移率族蛋白B1(HMGB1)、视黄醇结合蛋白4(RBP4)、基质金属蛋白酶-1(MMP-1)、组织抑制因子-1(TIMP-1);同时实验组于术后给予孕三烯酮治疗,观察服药1,3个月后患者血清HMGB1、RBP4、MMP-1、TIMP-1变化情况。结果实验组与对照组比较,血清HMGB1、RBP4、MMP-1、TIMP-1均高于对照组;同时实验组给予药物治疗后血清HMGB1、RBP4、MMP-1、TIMP-1与治疗前相比较下降明显,差异有统计学意义(P<0.05)。结论腹腔镜联合孕三烯酮治疗卵巢子宫内膜异位囊肿,能有效降低患者血清HMGB1、RBP4、MMP-1、TIMP-1从而可作为该疾病较有效治疗方案。

Objective To study the effectiveness of laparoscope surgery combined with gestrinone on the serum levels of high-mobility group protein B1( HMGB1),retinol binding protein 4( RBP4),matrix metalloproteinase-1( MMP-1) and matrix tissue inhibitor of metalloproteinase-1( TIMP-1) in patients with endometriosis and its clinical significance. Methods A total of 68 patients with endometriosis( observation group) and 70 healthy women( control group) in our hospital from January 2012 to December 2013 were enrolled in this study. Gestrinone was administrated after the operation in the observation group. The serum levels of high mobility group protein B1( HMGB1),retinol binding protein 4( RBP4),matrix metalloproteinase-1( MMP-1) and matrix tissue inhibitor of metalloproteinase-1( TIMP-1) of two groups were tested by ELISA before and after the treatment. Results The serum level of HMGB1,RBP4,MMP-1 and TIMP-1 in the observation group were higher than those in the control group; There had a drop of HMGB1,RBP4,MMP-1 and TIMP-1 after laparoscopic surgery combined with gestrinone,there was a significantly difference( P < 0. 05). Conclusion Laparoscopic surgery combined with gestrinone can decrease the serum level of HMGB1,RBP4,MMP-1 and TIMP-1,and should be a better choice of effective regimens.
