MDHAQ用于类风湿关节炎患者病情评估的临床价值分析

Clinical Value of MDHAQ for Disease Activity Assessment in Patients with Rheumatoid Arthritis

目的分析多维健康评估问卷(MDHAQ)用于类风湿关节炎(RA)患者病情评估的临床价值。方法纳入100例RA患者,由患者独立完成MDHAQ,计算患者数据指标常规评估值(RAPID),结合临床上对患者的关节评估结果及其它临床资料,分析RAPID3与28关节疾病活动指数(DAS28)、临床疾病活动指数(CDAI)、简化的疾病活动指数(SDAI)以及疾病特征的相关性。结果由MDHAQ计算所得的RAPID3与血沉、C反应蛋白、肿胀关节数、压痛关节数以及医生对疾病的总体评估均显著相关(P<0.05)。Cronbach’sα系数0.821,KMO值为0.576,Bartlett球形检验P<0.001,RAPID3与DAS28、CDAI、SDAI的相关系数分别为0.743、0.697和0.640(P<0.001)。结论 MDHAQ用于RA的疾病活动度评估具有较好的信度及效度,且操作简便快捷,适用于繁忙的临床实践。

Objective To evaluate the clinical value of the mutidimensional health assessment questionnaire（MDHAQ）for disease activity assessment in patients with rheumatoid arthritis（RA）.Methods A total of 100 RA patients were recruited in this study and completed the MDHAQ.Clinical data and involved joint function assessment were also collected.Routine assessment of patient index data 3（RAPID3）was calculated using the MDHAQ.The correlations between RAPID3 and disease activity score 28（DAS28）,simplified disease activity index（CDAI）,simplified disease activity index（SDAI）and clinical characteristics were analyzed.Results RAPID3 was correlated with erythrocyte sedimentation rate（ESR）,C-reactive protein（CRP）,swollen joints count,tender joints count and overall physician rating（P〈0.05）.The MDHAQ had a Cronbach＇sαcoefficient of 0.821 and a KMO of 0.576.Bartlett＇s test of sphericity showed P〈0.001.RAPID3 was also correlated with DAS28,CDAI and SDAI,with a coefficient of 0.743,0.697 and 0.640（P〈0.001）,respectively.Conclusion MDHAQ is a reliable instrument for disease activity assessment in patients with RA.It is easy to administer in busy clinical settings.
