德胜社区卫生服务中心四级康复模式的构建及实施效果分析

The Construction and Effectiveness of Four Levels Rehabilitation Model in Desheng Community Health Service Center

目的构建北京市德胜社区卫生服务中心四级康复模式,并探讨其实施效果。方法自2006年5月起在北京市德胜社区卫生服务中心开始构建并实行四级康复模式,截至2012年对区域内残疾与功能障碍人群康复需求及利用情况进行调查,并对2006—2008年脑卒中患者康复医疗效果及残疾人康复宣教结果进行评价。结果德胜地区脑卒中患者有医疗康复需求者共252人,其中接受康复训练的有205人(81.3%)。德胜地区残疾人有康复护理需求者75人,实施康复护理的有68人(90.7%),接受康复护理指导的有73人(97.3%)。康复医疗效果显示,脑卒中患者经正规康复治疗后,其活动功能及日常生活能力均高于对照组,差异有统计学意义(P<0.05)。康复宣教效果显示,残疾人康复知识掌握及行为改变情况在康复宣教后改善,与宣教前相比差异有统计学意义(P<0.05)。结论德胜社区卫生服务中心四级康复模式对残疾人康复效果明显,具有推广价值。

Objective To establish rehabilitation model with four levels and explore its effectiveness. Methods From May 2006, we began to construct and apply the rehabilitation model with four levels in Desheng community health service center. In 2012, we investigated the disability and dysfunction people in this area about their rehabilitation needs and the utilization situation of the mode and evaluated the rehabilitation effects in stroke patients and effects of rehabilitation education in the disabled population during the period of 2006 and 2008. Results 252 stroke patients in Desheng community needed medical rehabilitation and 205 （81.3%） already received the training. 75 disabled needed rehabilitation care, 68 （90. 7% ） received the care and 73 （97. 3% ） received rehabilitation care guide. Investigation on the effect of medical rehabilitation showed that in stroke patients who received formal rehabilitation training after treatment, their activity function and daily living activity increased significantly than those in the control group （ P 〈 0. 05 ） . Investigation on the effect of rehabilitation education showed that disabled people had a better mastery of rehabilitation knowledge and an improving change in behavior compared those before education and the differences were significant （P 〈 0. 05 ） . Conclusion Community rehabilitation model with four levels was efficient for helping more disabled people to get better rehabilitation.
