MR肩关节造影在肩袖损伤诊断中的价值

The value of MR shoulder arthrography in diagnosis of rotator cuff injury

目的：探讨MR肩关节造影在肩袖损伤诊断中的价值。方法选取本院2013年1月-2014年12月收治的肩袖损伤患者160例,术前均经常规MRI检查确诊并在手术中证实为肩袖损伤,其中85例接受MR造影检查,85例患者中有31例接受关节镜诊治。结果31例接受关节镜诊治的患者中,肩袖全层撕裂8例,MRI诊断准确率为62.50豫,MR造影诊断准确率为100.00豫；肩袖部分撕裂19例,MRI诊断准确率为57.89豫,MR造影诊断准确率为100.00豫；肩袖变性5例,MRI诊断准确率为40.00豫,MR造影诊断准确率为100.00豫；肩袖撕裂并发盂唇损伤9例,MRI诊断准确率为44.44豫,MR造影诊断准确率为100.00豫,MR造影诊断准确率高于MRI,差异有统计学意义（P〈0.05）。结论 MR造影诊断肩袖损伤的准确率高于MRI。

Objective To investigate the value of MR shoulder arthrography in diagnosis of rotator cuff injury. Methods 160 patients with rotator cuff injury admitted into our hospital from January 2013 to December 2014 were selected,and they were confirmed by conventional MRI preoperatively diagnosed and confirmed during surgery for rotator cuff injury, among them,85 patients were underwent MR angiography examination,among 85 patients,31 patients were underwent arthroscopic diagnosis and treatment. Results In 31 patients given arthroscopic diagnosis and treatment,there were 8 cases of full thickness rotator cuff tear,diagnostic accuracy rate of MRI was 62.50%,diagnostic accuracy rate of MR an-giography was 100.00%;there were 19 cases of partial tear of the rotator cuff,diagnostic accuracy rate of MRI was 57.89%,diagnostic accuracy rate of MR angiography was 100.00%;there were 5 cases of rotator cuff degeneration,diag-nostic accuracy rate of MRI was 40.00%,diagnostic accuracy rate of MR angiography was 100.00%;there were 9 cases rotator cuff tear complicated with labrum injury,diagnostic accuracy rate of MRI was 44.44%,diagnostic accuracy rate of MR angiography was 100.00%; diagnostic accuracy rate of MR angiography was higher than that of MRI,the difference was statistically significant （P〈0.05）. Conclusion The accuracy rate of MR angiography in the diagnosis of rotator cuff injury is higher than MRI.
