妊娠期高血压疾病患者血清瘦素水平的变化及意义

The clinical significance of serum leptin level in patients with hypertensive disorders in pregnancy

目的：研究瘦素在妊娠期高血压疾病患者血清中的表达及其在妊娠期高血压疾病发病中的作用。方法收集2012年10月-2014年6月在粤北人民医院住院的正常孕妇、妊娠期高血压患者、轻度子痫前期患者、重度子痫前期患者各20例，抽取空腹静脉血用放射免疫法测定血清瘦素含量。结果各组孕妇的血清瘦素水平与收缩压及舒张压均呈正相关（P〈0.05），与重度子痫前期组相关性更明显（r=0.70）。各组孕妇的血清瘦素水平与体重指数均呈正相关（P〈0.05），与对照组相关性更明显（r=0.69）。对照组、妊娠期高血压、轻度子痫前期、重度子痫前期组孕妇的血清瘦素含量分别为（20.1±8.1）、（20.2±8.4）、（28.8±8.1）、（40.6±10.3）ng/ml。对照组与妊娠期高血压组孕妇的血瘦素水平比较，差异无统计学意义（P〉0.05）；重度子痫前期组较轻度子痫前期组、妊娠期高血压组孕妇的血瘦素水平均增高，差异有统计学意义（P〈0.05）；轻度子痫前期组较妊娠期高血压组孕妇的血瘦素水平增高，差异有统计学意义（P〈0.05）。结论瘦素参与妊娠期高血压疾病的发生、病情发展，并且在疾病进展中有升高的倾向，瘦素是妊娠期高血压疾病的发病机制之一。

Objective To study the relationship between serum leptin level and the generation with hypertensive disor-ders in pregnancy. Methods Normal pregnant women （20 cases）,gestational hypertension patients （20 cases）,mild preeclampsia （20 cases）,severe preeclampsia （20 cases） in our hospital from October 2010 to June 2014 were studied by measuring serum leptin content with radioimmunoassay. Results Maternal serum leptin levels and systolic and diastolic blood pressure of each group was positively correlated （P〈0.05）,and the relationship of severe preeclampsia group was more obvious （r=0.70）.maternal serum leptin levels and BMI of each group was positively correlated （P〈0.05）,and the relationship of normal pregnancy group was more obvious （r=0.69）.Maternal serum leptin levels of normal pregnancy, gest-ational hypertension,mild preeclampsia,severe preeclampsia was （20.1±8.1） ng/ml,（20.2±8.4） ng/ml,（28.8±8.1） ng/ml, （40.6±10.3） ng/ml,respectively.Maternal leptin levels between normal pregnancy and gestational hypertension group showed no significant difference （P〉0.05）;Leptin was significantly increased in severe preeclampsia group than that of the mild preeclampsia group and gestational hypertension group （P〈0.05）.Leptin levels also increased in mild preeclampsia group than that of gestational hypertension group,there was significant difference （P〈0.05）. Conclusion Leptin is in-volved in generation,disease progression with hypertensive disorders in pregnancy,and it has a tendency to increase in disease progression.Leptin is one of the pathogenesis of hypertensive disorders in pregnancy.
