彩色多普勒超声检查在妇女早期乳腺癌筛查中的应用价值

Application value of color Doppler ultrasonography for diagnosis of early stage breast cancer

目的探讨彩色多普勒超声检查在妇女早期乳腺癌筛查中的应用价值。方法选取本院的5451例乳腺体检者作为研究对象,应用超声检查乳腺情况。对5451例乳腺体检者进行随访,对超声检查结果和病理检查结果进行分析。结果 5451例乳腺体检者中,乳腺异常2183例(40.0%),其中乳腺小叶增生1495例(27.4%)、乳腺纤维腺瘤567例(10.4%)、乳腺导管扩张109例(2.0%)、乳腺癌12例(0.2%)。随访1年,在663例来院经穿刺检查或手术切除肿物组织者中,B超检查显示乳腺癌12例,良性肿瘤651例;病理活检检查结果显示良性肿瘤616例,乳腺癌8例。与病理组织活检检查结果比较,良性肿瘤的超声诊断敏感度、特异度、准确度、阳性预测值和阴性预测值分别为99.4%(612/616)、17.0%(8/47)、93.5%(620/663)、94.0%(612/651)和66.7%(8/12),乳腺癌的超声诊断敏感度、特异度、准确度、阳性预测值和阴性预测值分别为100.0%(8/8)、99.4%(651/655)、99.4%(659/663)、66.7%(8/12)和100.0%(651/651)。结论彩色超声多普勒检查在乳腺癌早期发现、早期诊断中具有重要的临床应用价值。

Objective To evaluate the application value of color Doppler ultrasonography for diagnosis of early stage breast cancer. Methods 5451 women of breast examination were selected in our hospital as the research object,and were examined by ultrasonography.Follow up was done in 5451 cases of breast examination,the ultrasound findings and pathological examination results was analyzed. Results In 5451 cases,there were 2183 cases （40.0%） with abnormity, included 1495 cases （27.4%） of mammary lobular hyperplasia,567 cases （10.4%） of fibroadenoma,109 cases （2.0%） of mammary duct dilatation,12 cases （0.2%） of carcinoma. Follow-up of 1 year,in 663 cases of the biopsy or operation resection of tumor,B ultrasound examination showed,there were 12 cases with breast cancer,651 cases with benign tumor.Pathological biopsy resultsshowed,there were 616 cases with benign tumors,8 cases with breast cancer.The sensitivity,specificity,positive predictive values and negative predictive values,accuracy rate of benign breast lesions diagnosis was 99.4%（612/616）,17.0%（8/47）,93.5%（620/663）,94.0%（612/651） and 66.7%（8/12）.The sensitivity,specificity, positive predictive values and negative predictive values,accuracy rate of breast cancer diagnosis were 100.0%（8/8）,99.4%（651/655）,99.4%（659/663）,66.7%（8/12） and 100.0%（651/651）. Conclusion Color Doppler ultrasound has important clinical value in the early detection and diagnosis of breast cancer.
