腹腔镜与开腹右半结肠癌根治术的效果比较

Comparative analysis of effect on laparoscopic and laparotomy radical resection of right colon cancer

目的探讨腹腔镜与开腹右半结肠癌根治术的临床效果。方法选取2013年3月-2014年12月在本院接受右半结肠癌根治术患者120例,根据手术治疗方案将其分为腹腔镜手术治疗组（n=60）与开腹手术治疗组（n=60）。两组患者在接受手术治疗前进行各项指标检查,直至各项指标满足手术适应证,并严格遵循肿瘤根治原则。观察两组的治疗效果以及手术相关各项指标。结果两组的标本切除长度、结肠上旁淋巴结清扫数量、系膜间淋巴结清扫数量、血管根部淋巴结清扫数量比较差异无统计学意义（P〉0.05）。两组的手术时间比较差异无统计学意义（P〉0.05）。腹腔镜手术治疗组出血量为（106.34±32.25）ml,少于开腹手术治疗组（P〈0.05）;腹腔镜手术治疗组排气时间为（2.21±0.52）d,短于开腹手术治疗组（P〈0.05）;腹腔镜手术治疗组并发症发生率为8.33%,低于开腹手术治疗组（P〈0.05）。结论腹腔镜与开腹右半结肠癌根治术的临床疗效相当,但腹腔镜治疗术中创伤小,且术后恢复更快,值得临床推广。

Objective To analyze clinical effect of laparoscopic and laparotomy radical resection of right colon cancer. Methods 120 patients underwent radical hysterectomy of right colon cancer in our hospital from March 2013 to De- cember 2014 were selected and divided into treatment group of laparoscopic operation（n=60）and treatment group of la- parotomy operation（n=60）according to the operation treatment.Two groups of patients were examined in an operation in- dexe before treatment,until all indexes meet the operation indication,and strictly followed the principle of radical tumor. Therapeutic effect and operation related indexe between two groups were observed. Results The resented specimen length,number of lymph node dissection on the colon,number of omnipresent lymph node dissection,the number of lymph vessels roots had no statistical difference between two groups（P〉0.05）.There was no statistical difference in oper- ation time between two groups（P〉0.05）.Bleeding volume in treatment group of laparoscopic operation was（106.34±32.25） ml,less than that in treatment group of laparotomy operation （P〈0.05）.Exhaust time in treatment group of laparoscopic operation was（2.21±0.52） d,shorter than that in treatment group of laparotomy operation（P〈0.05）.Incidence of complica- tion intreatment group of laparoscopic operation was 8.33%,lower than that in treatment group of laparotomy operation （P〈0.05）. Conclusion Clinical efficacy of laparoscopic and laparotomy colorectal resection is quite right,laparoscopic surgery has less invasive and faster postoperative recovery,is worthy of promotion in clinic.
