319例男性同性恋HIV感染者/AIDS患者血浆病毒载量及T淋巴细胞亚型表达的回顾分析 被引量:3

摘要 目的:观察同性恋HIV感染者/AIDS患者血浆中HIV-1病毒载量、T淋巴细胞亚群变化。方法:HIV病毒载量检测采用COBASAmpliprep/COBASTaqman/COBASTaqman48 HIV,v2.0试剂盒,仪器采用COBASAmpliprep/COBASTaqman/COBASTaqman48。T淋巴细胞亚群检测采用FACSCalibur流式细胞仪,BD公司Multiset试剂盒。结果:病毒载量95.3%个体在5.1E+01拷贝·ml-1至1.0E+07拷贝·ml-1(中位数为1.5E+05拷贝·ml-1);CD3+细胞增高20例(6.26%)中有51%呈梯度减少(中位数998个·μl-1;CD4+细胞﹤400个·μl-1108例(33.85%),﹤300个·μl-1112例(35.10%),<100个·ul-169例(21.63%);CD8+细胞样本中有95%个体绝对值均在参考范围内,但其百分率均有不同程度增高;CD4+/CD8+之值除5例(1.56)>1.0外,其余均﹤1.0。临床表现无特殊。结论:319例男性同性恋HIV感染/AIDS患者的病毒载量、T淋巴细胞亚群变化及临床表现无异于其他感染途径者;CD8+细胞百分率意义大于绝对值。HIV-1分型是同性恋病毒分子流行病学的着眼点。 Objective: To observe the fluctuation of HIV-1 viral load and T lymphocyte subset count in plasma of homosexual infected with HIV and AIDs. Methods: HIV viral load was detected by using German COBASAmpliprep/ COBASTaqman / COBASTaqman48 and COBASAmpliprep/ COBASTaqman / COBASTaqman48 HIV,v2. 0 kit. T lymphocyte subset count detection was performed by FACSCalibur system and German BD Multiset kit. Results: It was from 5. 1E + 01 copies·ml- 1to 1. 0E + 07 copies·ml- 1in 95. 3% of the value of viral load,and the median was 1. 5E + 05 copies·ml- 1. There were 20 cases( 6. 26%) with CD3+count raised,among which there were 51% decreased in gradient way,the median was 998 μl- 1. CD4+cell count was less than 400 μl- 1and 300 μl- 1and 100 μl- 1in 108 cases( 33. 85%),112 cases( 35. 10%) and 69 cases( 21. 63%) respectively. 95% of CD8 absolute count was in reference range,while its percentage increased in different level. Except for 5 cases, CD4+/ CD8+was less than 1. 0. There was no special manifestation.Conclusion: There are no differences in viral load,T lymphocyte subset count and manifestation between 319 male homosexual infected with HIV and AIDs patients and patients infected through other route. There is more significance in CD8+percentage than its absolute count. HIV-1 subtype is a starting point in homosexual virus molecular epidemiology.
