摘要
目的探讨锁定钢板内固定联合抗骨质疏松治疗老年肱骨近端骨折的效果。方法 2011年2月-2014年8月河北联合大学附属医院收治的老年肱骨近端骨折患者55例随机分为治疗组（28例）和对照组（27例）。治疗组患者给予锁定钢板内固定治疗及3个月的抗骨质疏松治疗。对照组仅采用锁定钢板内固定治疗。比较两组患者住院时间、抗骨质疏松治疗后的骨密度及肩关节功能的恢复情况。结果与对照组比较,治疗组住院时间明显缩短[（7.0±0.9）d比（10.1±0.7）d],差异有统计学意义（P〈0.05）。治疗组和对照组患者术后3个月L2-4椎体骨密度分别为（0.80±0.11）、（0.64±0.04）g/cm^2,差异有统计学意义（P〈0.05）。治疗组肩关节功能的恢复情况：优15例、良10例、可2例、差1例;对照组：优10例、良10例、可4例、差3例,两组比较差异有统计学意义（P〈0.05）。结论老年肱骨近端骨折采用锁定钢板内固定联合抗骨质疏松治疗临床效果好,住院时间短,肩关节功能恢复良好,值得临床推广应用。
Objective To disscuss the effect of the locking plate fixation with anti-osteoporosis drug treating proximal humeral fractures. Methods From February 2011 to August 2014, in Affiliated Hospital of Hebei United University, 55 patients with proximal humeral fractures were selected and divided into treatment group（28 cases） and control group（27 cases）. The treatment group was given locking plate fixation and anti osteoporosis treatment for 3 months. The control group was given just locking plate fixation. The hospitalization time, recovery of shoulder joint function and bone mineral density of the two groups were observed and compared. Results Compared with the control group, the hospitalization time of treatment group was significantly reduced [（7.0±0.9） d vs（10.1±0.7） d], the difference was statistically significant（P〈0.05）. L2-4bone mineral density after 3 months of two groups were（0.80±0.11）,（0.64±0.04） g/cm^2, the difference was statistically significant（P〈0.05）. Recovery of shoulder joint function of treatment group was 15 cases in excellent, 10 cases in good, 2 cases in fair, 1 case in poor, and the efficacy of the control group was 10 cases in excellent, good in 10 cases, 4 cases in fair and 3 cases in poor, the difference was statistically significant（P〈0.05）. Conclusion The effect of locking plate fixation with anti-osteoporosis drug is better, the hospitalization time is shorter,shoulder joint function recovers well, it is worthy of clinical application.
