老年人社区获得性肺炎的临床特点及诊治分析

The clinical characteristics of elderly patients with community acquired pneumonia and analysis of diagnosis and treatment

目的探讨老年社区获得性肺炎(CAP)的临床特点。方法分析110例老年CAP住院患者的临床资料,根据年龄分为高龄组与非高龄组,分析两组患者的临床表现、辅助检查及治疗转归。结果与非高龄组患者比较,高龄组CAP患者临床表现不典型,肺部体征明显,病变范围较广。白细胞总数增高不明显,而血清白蛋白及PO2降低明显,痰病原菌的阳性率也明显高于非高龄组。结论重视高龄CAP的不典型临床症状及特点,对高龄CAP患者做到早诊断、早治疗,合理使用抗生素及营养支持等综合治疗是提高治愈率的关键。

Objective To discuss the clinical characteristics of elderly patients with community acquired pneumonia（ CAP）. Methods Summary and analysis 110 cases clinical date of inpatients with CAP,comparative analysis of elder group and Non-elder group＇s clinical characteristics,assistant examination,diagnosis and treatment. Results Compared with Non-elder group,Clinical characteristics and laboratory examination of elderly patients with community acquired pneumonia were atypical and lung signs were obviously,total white cell count level increased not obvious,but ALB and PO2 decreased obviously,detection rate of pathogenic bacteria of sputum was higher. Conclusion According to the atypical clinical characteristics of elderly patients with CAP,the elder should be early diagnosised and early treatmented,and the key for improving the cure rate is reasonable using antibiotic and comprehensive treatment.
