经颅彩色多普勒超声联合超声造影诊断颅内动脉瘤的价值 被引量:4

摘要 目的评价经颅彩色多普勒超声(TCCS)联合超声造影诊断颅内动脉瘤的临床价值。方法 38例疑诊为颅内动脉瘤患者,TCCS联合超声造影观测其颅内动脉的走行、内径、动脉瘤大小及其血流动力学参数,并与DSA对比。结果 38例患者,与DSA对比,TCCS诊断颅内动脉瘤敏感性63.16%,特异性89.47%,准确性76.32%,阳性预测值85.71%,阴性预测值70.83%;TCCS联合超声造影诊断颅内动脉瘤敏感性78.95%,特异性89.47%,准确性84.21%,阳性预测值88.23%,阴性预测值80.95%。结论 TCCS联合超声造影可以提高颅内动脉瘤诊断敏感性和准确性,为临床提供一种无创、简便、实时的筛查方法。 Objective To evaluate the clinical value of transcranial color-coded sonography(TCCS) combined with contrast-enhanced ultrasonography(CEUS)in diagnosis of intracranial aneurysms. Methods TCCS and CEUS were applied on thirty-eight patients suspected with intracranial aneurysms. The course and internal size of the intracranial arteries were observed, the size and the hemodynamic parameters of the aneurysms were recorded. All results of ultrasound were compared with those of those DSA. Results Compared with DSA, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of TCCS were 63.16%,89.47%,76.32%, 85.71% and 70.83%, respectively, those of TCCS combined with CEUS were 78.95%, 89.47%, 84.21%, 88.23%, 80.95%, respectively. Conclusion The sensitivity and accuracy in diagnosis of intracranial aneurysms can be improved by combining TCCS with CEUS, which can be chosen as a non-invasive,simple, real-time method in clinic.
