彩色多普勒超声在小儿阴囊急症鉴别诊断及治疗中的应用价值

Applied value of color Doppler ultrasound in differential diagnosis and treatment of acute scrotum in children

目的探讨彩色多普勒超声在小儿阴囊急症鉴别诊断及治疗中的应用价值。方法回顾性分析2008-2014年收治的200例小儿阴囊急症的临床资料及超声表现,并与手术病理及随访结果对照。结果超声诊断符合率93.2%。200例中,睾丸附件扭转70例,超声表现为在睾丸与附睾头之间或睾丸旁出现异常结节,结节内部无血供,而结节周围的结构血供增多;急性附睾睾丸炎60例,超声表现为受累结构肿大,回声不均,血供明显增多;睾丸扭转18例,超声表现为精索走行迂曲或扭曲呈团,睾丸血运明显减少或消失;其他52例。结论彩色多普勒超声在小儿阴囊急症的鉴别诊断及治疗中具有重要价值,是临床首选的检查方法。

Objective To explore the applied value of color Doppler ultrasound in differential diagnosis and treatment of acute scrotum in children. Methods From 2008 to 2014,the clinical data and ultrasonography findings of 200 children with acute scrotum were retrospectively analyzed,and the results were compared with the surgical pathology and follow-up results. Results The coincidence rate of ultrasound diagnosis was 93. 2%. Of 200 cases,there were 70 cases of testicular appendage torsion,60 cases of acute epididymo-orchitis,18 cases of testicular torsion and 52 cases of other situations. The ultrasound of 70 cases of testicular appendage torsion showed that abnormal nodules without blood supply between testicle and head of epididymis or near the testicle,and the increased blood flow in surrounding structures of the nodule. The ultrasound of 60 cases of acute epididymo-orchitis showed the swelling affected structures,inhomogeneous echo,and the increased blood flow. The ultrasound of 18 cases of testicular torsion showed that the spermatic cord was distorted,and the testicular blood supply significantly reduced or disappeared. Conclusion Color Doppler ultrasound is of great value in the differential diagnosis and treatment of acute scrotum in children,and it is the preferred clinical examination method.
