关节镜与MRI在诊断膝关节韧带损伤的作用研究

Comparing study on MRI diagnosis and joint screen diagnosis of knee joint ligament trauma

目的探讨关节镜与MRI对膝关节韧带损伤的诊断价值,并对手术的指导作用。方法总结分析26例,其中,13例直接作关节镜,13例先做MRI,再作关节镜手术,观察MRI对手术治疗的指导意义。结果 26条膝关节损伤的韧带中,患者膝关节前交叉韧带发生损伤11例,患者后交叉韧带发生损伤3例,患者内侧副韧带发生损伤9例,外侧副韧带损伤3条。单纯关节镜手术时间和出血量分别为(1.20±0.30)h和(156±1.60)ml,MRI后再作关节镜手术时间和出血量分别为(0.66±0.23)h和(62±1.23)ml,P<0.05,两者差异有统计学意义。结论 MRI是诊断膝关节韧带损伤的理想非创伤性检查方法,对关节镜手术有指导意义。

Objective To investigate the diagnostic value of achilles tendon rupture by MR images and compare with joint screen in diagnosing injury of acute knee trauma. Methods 26 cases with acute knee trauma were confirmed ligament injury by operation or joint-screen examination. The ligament injuries included tear and disruption. Results 26 ligaments injuries were found in all cases, including 11 anterior crossing ligament（ CL）, 3 posterior CL, 9 medical CL and 3 lateral CL. The time and amount of bleeding of Simple arthroscopic surgery is 1.20 ±0.30h and 156 ± 1.60ml, The time and amount of bleeding of Simple arthroscopic surgery after MRI is 0.66 ± 0.23 h and 62± 1.23 ml. Conclusions MRI is a kind of reliable, noninvasive way for diagnosisl igament injury in acute knee trauma.
