先天性心脏病患儿父母疾病不确定感与焦虑、抑郁的关系

Relationship between uncertainty in illness,anxiety,and depression in parents of children with congenital heart disease

目的:探讨先天性心脏病(CHD)患儿父母疾病不确定感与焦虑、抑郁的关系。方法:采用问卷调查法,应用中文版疾病不确定感父母量表(PPUS)和医院焦虑、抑郁量表(HADS)对113例CHD患儿父母进行测评。结果:CHD患儿父母的疾病不确定感总分为(82.62±11.02)分,焦虑总分为(11.48±3.67)分,抑郁总分为(10.35±4.23)分,疾病不确定感总分及不明确性、信息缺乏性、不可预测维度与焦虑呈正相关;疾病不确定感总分及不明确性、复杂性、不可预测性维度与抑郁呈正相关(P<0.05)。回归分析显示,不明确性、家庭人均月收入和信息缺乏性是患儿父母焦虑的影响因素(β=0.596、-0.240和0.193,P<0.05);不明确性、家长性别是抑郁的影响因素(β=0.274和0.226,P<0.05)。结论:CHD患儿父母存在较高程度的疾病不确定感、焦虑和抑郁,临床工作中应对患儿父母的疾病不确定感进行评估并给予个体化干预,通过降低疾病不确定感,缓解焦虑和抑郁情绪。

Aim：To explore the relationship between uncertainty in illness , anxiety, and depression in parents of chil-dren with congenital heart disease .Methods：A total of 113 parents of children with congenital heart disease were recruited for the investigation .A descriptive cross-sectional study was conducted by using Parents＇Perception of Uncertainty Scale （ PPUS ） and Hospital Anxiety and Depression Scale （ HADS ） .Results： The total score of uncertainty in illness was （82.62 ±11.02）,that of anxiety was （11.48 ±3.67）,while that of depression was （10.35 ±4.23）.Uncertainty in illness and the dimensions of ambiguity, lack of information and unpredictability were positively associated with anxiety （P〈0.05）;uncertainty in illness and the dimensions of ambiguity ,complexity and unpredictability were positively associated with depres-sion（P〈0.05）.Multiple linear regression showed that ambiguity , average monthly income and lack of information were influ-ential factors of parents＇anxiety（β=0.596,-0.240 and 0.193,P〈0.05）;ambiguity and parents＇gender were influential fac-tors of parents＇depression（β=0.274 and 0.226,P〈0.05）.Conclusion：The uncertainty in illness,anxiety and depression in parents of children with congenital heart disease are at a higher level .It is necessary for medical staff to evaluate the uncer-tainty in illness and provide customerized intervention for parents in clinical practice so as to decrease the uncertainty in ill -ness and relieve the anxiety as well as depression .
