新疆维吾尔自治区2012年脊髓灰质炎病毒学监测结果和分析 被引量:8

摘要 目的评价新疆维吾尔自治区(新疆)2012年脊髓灰质炎(脊灰)病毒学监测情况。方法按照世界卫生组织(World Health Organization,WHO)第4版《脊灰实验室手册》操作规程进行病毒分离、鉴定,对新疆2012年报告的儿童和成人急性弛缓性麻痹(Acute Flaccid Paralysis,AFP)病例及非AFP病例、接触者粪便标本及环境污水标本进行脊灰病毒学监测和结果分析,由中国疾病预防控制中心(Center for Disease Control and Prevention,CDC)病毒病预防控制所国家脊灰实验室(National Polio Laboratory,NPL)对脊灰病毒(Poliovirus,PV)衣壳蛋白(Capsid Protein)VP1编码区进行核苷酸序列测定和分析,对分离到的PV株做型内鉴定(Intratypic Differentiation,ITD)。结果 2012年,新疆CDC脊灰实验室各项监测指标均达到卫生部和WHO的要求,共收到儿童和成人AFP病例及非AFP病例、接触者粪便标本1227份,环境污水标本12份。从儿童AFP病例粪便标本中分离到PV 14株,分离率为5.6%;非脊灰肠道病毒28株,分离率为11.3%;从环境污水标本中分离到PV 104株。所有PV经NPL进行ITD,均为疫苗相关株。结论 2012年新疆CDC脊灰实验室运转正常,在成人和儿童AFP病例及环境污水中未发现脊灰野病毒和疫苗衍生脊灰病毒,恢复了无脊灰状态。 Objective To evaluate the performance of virological monitoring of polio in Xinjiang Uygur Autonomous Region (Xinjiang), 2012. Methods We analyzed results of virological detection in feces samples and sewage samples from children and adult acute flaccid paralysis (AFP) cases and non-AFP contact cases reported in Xinjiang 2012, in accordance with the World Health Organization' s (WHO) fourth edition of Polio Laboratory Manual operating rules for virus isolation and identification. The Nation- al Polio Laboratory (NPL) of the National Institute for Viral Disease Control and Prevention of the Chi- nese Center for Disease Control and Prevention (CDC) determined and analyzed nucleotide sequence of VPI coding region of poliovirus (PV) capsid protein, and intratypic differentiation (ITD) for PV strains. Result In 2012, all monitoring indicators reached the requirements of the Ministry of Health arrd WHO. In total, there were 1,227 stool specimens from children and adult acute flaccid paralysis (AFP) cases and non-AFP contacts, and 12 environmental sewage samples. There were 14 PV strains and 28 strains non-polio enterovirus (NPEV) isolated from children's AFP feces samples with rates of 5.6% and 11. 3%, respectively; 104 PV strains were isolated from environmental sewage samples. All PVs were sent to NPL for typing identification, and the results showed all to be vaccine-related strains. Conclusion In 2012, the Xinjiang CDC polio laboratory was operating normally; no wild poliovirus was found in adult or child AFP cases or in environment sewage,consistent with polio-free status.
