摘要
目的探讨和研究彩色多普勒超声用于肾动脉狭窄的诊断价值。方法 58例肾动脉狭窄患者,均采用彩色多普勒超声进行检查,对肾动脉收缩期峰值流速(PSV)、肾内段动脉阻力指数(RI)、肾脏长轴等进行测定,以肾动脉造影结果为对比参考,评价彩色多普勒超声用于肾动脉狭窄诊断的准确性。结果本次58例患者中共113根肾动脉进行检查,与肾动脉造影结果相比,敏感性60.0%,特异性83.3%,阳性预测值83.0%,阴性预测值60.6%;将113根肾动脉分为健侧组48根,患侧组65根,患侧组的肾脏长轴、PSV、RI与健侧组相比差异具有统计学意义(P<0.05)。结论肾动脉彩色多普勒超声检查对于肾动脉狭窄的诊断结果准确度较高,但敏感性一般,用于介入治疗后的疗效评价效果较好,值得在临床上推广和应用。
Objective To investigate and research the value of color Doppler ultrasound in diagnosis of renal artery stenosis. Methods Color Doppler ultrasound was used in the inspection of 58 cases with renal artery stenosis, and measurements was taken of the renal artery peak systolic velocity(PSV), renal artery resistance index(RI), long axis of kidney. The results of renal angiography were taken as the reference to evaluate the diagnostic accuracy of color Doppler ultrasound for renal artery stenosis. Results Among the58 cases, there were 113 renal arteries examined. Comparison with the renal angiography showed that sensitivity was 60.0%, specificity was 83.3%, positive predictive value was 83.0%, and negative predictive value was 60.6%. These 113 renal arteries were dived into health-side group with 48 arteries and affected-side group with 65 arteries. The affectedside group had statistically significant differences of long axis of kidney, PSV, and RI with the health-side group(P〈0.05). Conclusion Color Doppler ultrasound has high accuracy but general sensitivity in the diagnosis of renal artery stenosis. It can provide good effect for interventional therapy, and is worth clinical promotion and application.
