经肛门局部切除低位直肠肿瘤治疗效果和预后

Therapeutic effect and prognosis of transanal local excision for low rectal tumor

目的探讨经肛门局部切除低位直肠肿瘤的效果及预后。方法选取2011年12月至2013年12月来安阳市灯塔医院治疗的直肠癌患者36例,随机分为观察组与对照组。观察组给予经肛门局部切除治疗,对照组给予传统的经腹部切除手术治疗,术后观察两组患者的临床疗效及其预后。结果观察组在手术时间、术后疼痛视觉VAS评分、住院时间、围术期并发症(腹腔感染、肺部感染、切口感染、吻合口瘘、术后肠梗阻等)等方面均优于对照组,且差异有统计学意义(P<0.05)。观察组均无围术期死亡及远处转移患者,局部复发3例,对照组无远处转移患者,但有7例患者在围术期发生了较为严重的并发症,死亡及局部复发各1例。结论经肛门局部切除治疗低位直肠肿瘤,其临床效果优于传统方法,安全性较高,值得在临床上推广应用。

Objective to investigate the cLinicaL therapeutic effect of transanaL LocaL excision on Low rectaL cancer and the prognosis. Methods From December 2011 to December 2013,36 patients with rectaL cancer in our hospitaL were randomLy di-vided into the observation group and the controL group. the patients in the observation group were given transanaL LocaL excision, and the patients in the controL group were treated by abdominaL resection operation. the cLinicaL effects and prognosis of the two groups was compared. Results the observation group was superior to the controL group in the operation time,postoperative pain score of visuaL VaS,hospitaLization time,peri-operative compLications（ abdominaL infection,puLmonary infection,incision infec-tion,anastomotic fistuLa,postoperative intestinaL obstruction）,and the differences were significant（P〈0. 05）. the observation group had no peri-operative period death or distant metastasis,LocaL recurrence occurred in 3 cases,the controL group had no me-tastasis,but 7 patients deveLoped severe compLications in peri-operative period,there was 1 case of death and 1 case of LocaL re-currence. Conclusion the cLinicaL effect of transanaL LocaL excision on Low rectaL tumor is better than that of traditionaL method, it has high safety,so is worthy of cLinicaL appLication.
