关节镜引导下微创松解术治疗创伤性膝关节僵直疗效分析

Curative Efficacy of Minimally Invasive Release Operation Guided by Arthroscopy in the Treatment of Post Traumatic Knee Stiffness

目的：探讨关节镜引导下微创松解术治疗创伤性膝关节僵直的临床疗效。方法回顾性分析2012年1月～2013年12月我科收治的行关节镜引导下微创松解术治疗的54例创伤性关节僵直患者的临床资料。观察手术时间、术中出血量等手术情况、膝关节功能优良率、关节活动度及并发症发生情况。结果54例患者均手术成功，手术时间（87.3±9.7）min，术中出血量（32.1±7.9）ml。随访1年，膝关节功能优34例（63.0%），良18例（33.3%），可1例（1.8%），差1例（1.8%），总优良率为96.3%，膝关节活动度由（38.8±9.4）°增加至（102.5±12.4）°。所有患者均未出现皮肤坏死、感染等并发症。结论关节镜引导下微创松解术是治疗创伤性膝关节僵直的有效手术方式，临床疗效高，且创伤小，并发症少，临床上值得进一步推广。

Objective To investigate the clinical effect of minimally invasive release operation guided by arthroscopy in the treatment of post traumatic knee stiffness. Methods54 cases of post traumatic knee stiffness from January, 2012 to December, 2013 who were admitted and given minimally invasive release operation guided by arthroscopy were selected as research objects. Then their clinical materials were retrospectively analyzed to observe operation situation of operation time and bleeding volume during the operation, excellent and good rate of knee joint function, range of motion of knee joint and incidence of complications.ResultsAll patients’ operation were successful. The operation time was （87.3±9.7） min and bleeding volume during the operation was （32.1±7.9） ml. After the one-year follow-up, the excellent and good rate of knee joint function was 96.3% and the range of motion of the knee joint increased from （38.8±9.4） °to （102.5±12.4） °. And there’s no case with complication of skin necrosis or infection.ConclusionMinimally invasive release operation guided by arthroscopy in the treatment of post traumatic knee stiffness is effective, which has high curative efifcacy with less trauma and complications.
