颈椎骨成熟度与头帽颈牵引矫治时机的选择

Timing of headgear treatment based on the maturation of cervical vertebra

目的 :根据颈椎骨成熟度 ,通过A点变化的研究 ,为临床采用头帽颈牵引矫治Ⅱ类错牙合最佳时机选择提供参考。方法 :通过观察 35例临床诊断为Ⅱ类 1分类错牙合且伴有面中部突出患者的X线头颅侧位定位片 ,依据O’Reilly制定的标准确定每位患者的骨龄。分析、比较不同颈椎骨发育阶段头帽牵引矫治前后A点的变化情况。结果 :不同颈椎骨发育阶段与达到最大矫治效果之间具有统计学意义 (P <0 .0 5 )。其中第二 ,三阶段A点的变化最为显著。结论 :选择S2 -3

Objective: To provide clinical reference for timing of headgear treatment on the basis of cervical vertebra maturation.Methods: 35 patients with Class Ⅱ, Division 1 malocclusions and midface prognathism were investigated by lateral cephalometric radiographs. Each record was assigned a skeletal age according to the method established by O'Reilly. The changes of point A were analyzed before and after headgear treatment at various cervial vertebra maturation stages.Results: Statistical significance was found at diffrent maturation stage when greater treatment changes could be obtained(P>0.05). At stage 2 to 3, the changes of point A was the greatest. Conclusion: Timing of headgear treatment based on the morphological variation of cervical vertebra can obtain the maximum desirable orthopedic effect.
