摘要 目的:探讨早期微创穿刺碎引流术治疗高血压所致的中小量基底核脑出血的效果,为临床早期中小量基底核脑出血的治疗提供依据。方法:选取68例高血压所致的中小量基底核脑出血患者,按入院顺序分为研究组和对照组各34例,对照组采用内科保守治疗,研究组在常规治疗基础上行早期微创穿刺碎引流术。比较2组患者治疗后3、14 d血肿、脑水肿体积和神经功能缺损评分;比较2组患者治疗后3个月日常生活能力。结果:研究组患者治疗后3、14 d的血肿体积和水肿体积均较对照组下降明显,同时其神经功能缺损评分也较对照组明显减少(P<0.01);治疗后3个月,研究组日常生活能力独立生活+轻度依赖的总有效率73.53%,高于对照组的44.12%(P<0.05)。结论:早期微创穿刺碎引流术治疗中小量基底核脑出血疗效安全可靠,值得推广应用。 Objective: To investigate the effects of early minimally invasive puncture drainage in the treatment of medium and minimum basal ganglia hemorrhage caused by hypertension,and provide theoretical basis for its clinical treatment. Methods: Sixty-eight patients with basal ganglia hemorrhage caused by hypertension were divided into the control group and study group( 34 cases each group) according to the order of admission. The control group and study group were treated with conservative treatment and early minimally invasive puncture drainage based on the conservative treatment,respectively. The hematoma and cerebral edema volume and neurological function scores between two groups were compared after 3 and 14 days of treatment,the abilities of daily life between two groups were analyzed after 3 months of treatment. Results: Compared with the control group,the hematoma and edema volume and neurological deficit scores in study group decreased significantly after 3 and 14 days of treatment( P 0. 01),the total effective rate of independent living and mildly dependent in study group( 73. 53%) was significantly higher than that in control group( 44. 12%) after 3months of treatment( P 0. 05). Conclusions: The treatment of medium and minimum basal ganglia hemorrhage caused by hypertension with minimally invasive puncture drainage is safe and reliable,which should be widely applied.
