血清Cys-C和Hcy联合检测对2型糖尿病肾病早期诊断的价值及其与氧化应激的关系 被引量:21

摘要 目的:研究血清胱抑素C(Cys-C)和同型半胱氨酸(Hcy)联合检测对2型糖尿病肾病早期诊断的价值及其与氧化应激的关系。方法:选取在本院就诊的2型糖尿病未合并肾功能损害的50例患者纳入对照组,合并肾功能损害Ⅰ期的50例纳入肾功能Ⅰ期组,合并肾功能损害Ⅱ期的50例纳入肾功能Ⅱ期组。检测3组肾功能指标以及氧化应激指标。结果:3组受试者的血肌酐、血尿素氮水平无差异,肾功能Ⅰ期组、Ⅱ期组血清Cys-C、Hcy含量高于对照组,差异有统计学意义(P<0.05);肾功能Ⅰ期组、Ⅱ期组的血清超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、过氧化氢酶(CAT)含量低于对照组,丙二醛(MDA)以及p38、MAPK、ERK、AMPK含量高于对照组,差异有统计学意义(P<0.05);血清SOD、GSH-Px、CAT含量与Cys-C、Hcy呈负相关,MDA以及p38、MAPK、ERK、AMPK含量与Cys-C、Hcy呈正相关。结论:血清Cys-C和Hcy检测有助于2型糖尿病肾病的早期诊断,且与氧化应激程度具有良好关系。 Objective: To study the value of serum Cys C and Hcy in early diagnosis of type 2 diabetic nephropathy and its correlation with oxidative stress. Methods: 50 cases of.type 2 diabetes patients with no renal function impairment were en- rolled in control group, 50 cases of type 2 diabetes patients with renal function impairment stage I were enrolled in renal func- tion stage I group, 50 cases of type 2 diabetes patients with renal function impairment stage Ⅱ were enrolled in renal function stage Ⅱ group. Then renal function intexes and oxidative stress index were detected. Results: (1) Renal function: serum creat- inine and blood urea nitrogen of three groups were not significantly different from each other (P〉0.05) ; Cys C, Hcy content of renal function stage I group and renal function stage 11 group were higher than those of control group; (2) Oxidative stress index: serum SOD, GSH-Px, CAT contents of renal function stage I group and renal function stage Ⅱ group were lower than those of control group; serum MDA, p38, MAPK, ERK and AMPK contents of renal function stage I group and renal func- tion stage [[ group were higher than those of control group; (3)Serum SOD, GSH-Px, CAT contents were negatively correla- ted with Cys C, Hcy content; serum MDA, p38, MAPK, ERK and AMPK contents were positively correlated with Cys C, Hey content. Conclusion.. Serum Cys C and Hcy detection is helpful for the early diagnosis of type 2 diabetic nephropathy and have good relationship with oxidative stress degree.
