纤维支气管镜在肺癌患者术前双腔气管导管定位中的应用

Application of Fiberoptic Bronchoscopy in Preoperative Positioning of Double-lumen Endobronchial Tube in Lung Cancer Patients

目的探讨纤维支气管镜在胸外科患者术前双腔气管导管定位的应用价值。方法选取80例行双腔支气管导管插管的肺癌患者,随机分为观察组和对照组,每组各40例。观察组纤维支气管镜引导下行双腔支气管导管插管,对照组采用传统的听诊法定位插管,随后应用纤维支气管镜精确定位。观察比较两组患者插管一次成功率、导管准确定位所需的时间以及术后气管插管相关并发症发生情况。结果观察组插管一次成功率、导管准确定位所需时间、SpO2下降情况、气管插管并发症发生情况均显著优于对照组,差异有统计学意义(P<0.05)。结论纤维支气管镜对肺癌患者术前双腔气管导管定位具有重要价值,它能提高插管一次成功率,减少插管时间,降低插管相关并发症的发生率,值得临床推广。

Objective To investigate the application value of fiberoptic bronchoscopy in preoperative double -lumen en-dobronchial tube positioning of lung cancer patients .Methods 80 cases of lung cancer patients who received double lumen endo-bronchial intubation were randomly divided into the observation group and the control group ,with 40 cases in each group .The ob-servation group received fiberoptic bronchoscopy guided double lumen bronchial intubation ,the control group received traditional auscultation positioning cannula ,and then the precise positioning was guided by fiberoptic bronchoscopy .Success rate of intuba-tion,time required for accurate positioning of the catheter and postoperative tracheal intubation related complications of the 2 groups were observed and compared .Results Success rate of intubation ,time required for accurate positioning of the catheter , SpO2 drop rate,endobronchial intubation complications of the observation group were significantly better than those of the control group,the difference was statistically significant （P〈0.05）.Conclusion Fiberoptic bronchoscopy has important value in preop-erative double lumen catheter positioning of lung cancer patients ,it can improve the success rate of intubation ,reduce the intuba-tion time,and reduce catheter related complications ,it is worthy of clinical application .
