佩戴义齿与头颈癌风险相关性的Meta分析

Association between Wearing Dentures and Risk of Head and Neck Cancer:A Meta-Analysis

摘要 目的采用Meta分析的方法探讨佩戴义齿与罹患头颈癌风险的相关性。方法计算机检索CNKI、Wan Fang Data、Pub Med和EMbase数据库,查找公开发表的探究佩戴义齿与头颈癌风险的队列研究与病例-对照研究,检索时限均为从建库至2014年4月20日。由2位评价员按照纳入与排除标准独立筛选文献、提取资料和评价纳入研究的方法学质量后,采用Comprehensive Meta-Analysis v 2.2软件进行Meta分析。结果最终纳入7篇文献共8个病例-对照研究。Meta分析结果显示佩戴义齿与头颈癌的发生存在低度相关性[OR=1.08,95%CI(1.00,1.16)];根据性别、癌症部位、是否校正混杂因素和研究发表时间的亚组分析结果均显示无显著相关性。结论佩带义齿与头颈癌的发生无明显相关性,且无性别及部位差异。鉴于本Meta分析的局限性,仍需开展大样本、高质量的研究来进一步论证。 Objective To explore the association between wearing dentures and the risk of head and neck cancer using meta-analysis. Methods Pub Med,EMbase,CNKI,and Wan Fang Data were searched up to April 30 th,2014,for cohort studies and case-control studies about the association between wearing dentures and the risk of head and neck cancer. Literature screening according to the inclusion and exclusion criteria,data extraction and methodological quality assessment of included studies were completed by two reviewers independently. Then meta-analysis was conducted using Comprehensive Meta-Analysis v 2.2 sot ware. Results Eight case-control studies in 7 reports were finally included. The results of meta-analysis revealed that,low-degree association existed between wearing dentures and the risk of head and neck cancer(OR=1.08,95%CI 1.00 to 1.16); and the results of further subgroup analysis(according to gender,cancer lesions,confounders adjustment,and publication years) also showed no significant difference. Conclusion Wearing dentures is not significantly associated with the risk of head and neck cancer(no difference regarding gender or cancer lesions). Due to limitations of this meta-analysis,high-quality studies with large sample size are needed to further verify the above conclusion.
